摘要
目的研究彩色多普勒超声与乳腺钼靶X射线影像在乳腺原位癌早期诊断中的应用。方法选择安康市旬阳县医院在2011年3月至2015年3月收治的60例乳腺原位癌患者为研究对象,对所有患者的临床资料进行回顾性分析。根据不同诊断方法将其分成A组和B组,分别为31例和29例。A组利用彩色多普勒超声诊断,B组利用乳腺钼靶X射线影像诊断。依据乳腺影像学报告、数据系统分级标准(BIRADS)与超声诊断(BIRADS US)进行分级,通过Cohen kappa统计方法对两种方法的分级符合度进行评估。结果彩色多普勒超声与乳腺钼靶X射线影像经BIRADS分级后,≥4级的结果基本一致,其中23例分级结果完全一致,差异无统计学意义(P>0.05)。结论彩色多普勒超声与乳腺钼靶X射线影像结果经BIRADS和BIRADS US分级后,分级的结果具有较高的一致性,表明二者对乳腺原位癌临床早期诊断均具有较高的应用价值,值得临床应用并推广。
Objective To study the application of color doppler ultrasonography(CDU) and molybdenum target X-ray images(MTXI) in early diagnosis of breast carcinoma in situ. Methods Sixty cases of patients with breast carcinoma in situ in Xunyang County Hospital from March 2011 to March 2015 were selected as the study subjects and randomly divided into group A of 31 cases and group B of 29 cases. Patients in the group A and group B were examined with CDU and MTXI respectively. The classification was performed on the basis of the BIRADS, BIRADS US and mammography reporting. The grading conformity degree of the two methods were evaluated according to statistical approach Cohen kappa.Results After classified with BIRADS, the results showed that, ≥4 level of CDU and MTXI were basically the same, with 23 cases consistented with classified results, the difference was not significant(P〉0.05). Conclusion The classification results of CDU and MTXI possesses high consistency and more application value in early diagnosis of breast carcinoma in situ, which indicates they could be widely promoted in clinical practice.
