腹部手术后粘连性肠梗阻的预防

目的探究腹部手术后粘连性肠梗阻的有效防治措施。方法选择本院2006年1月至2016年6月行腹部手术的200例患者为研究对象,将其分为观察组和对照组,各100例。对照组行常规西医治疗,观察组患者在此基础上连续使用大承气汤加减治疗3个月,通便后停药。对比两组患者停药后肠梗阻再发生率、肠胃功能时间恢复情况和治疗满意率。结果观察组患者的肠梗阻再发生率、肠道功能恢复时间和平均恢复时间均优于对照组(P<0.05);对照组患者治疗满意率为81.00%,明显低于观察组治疗满意率95.00%(P<0.05)。结论对于行腹部手术者,以连续服药3个月大承气汤的方式治疗,不仅可有效预防粘连性肠梗阻的发生,安全性强,简单实用,效果确切,还可有效预防二次手术,降低患者家属经济压力,非常适合基层医院,值得进一步临床推广。

Objective To explore the effective prevention and treatment measures of adhesive intestinal obstruction after abdominal operation. Methods Two hundred cases of abdominal surgery patients in our hospital from January 2006 to January 2016 were selected as the research objects, which were divided into observation group and control group, with 100 cases in each group. The control group received conventional western medicine, while the observation group was treated with Dachengqi decoction for three months on the basis of the control group until defecation. The incidence of intestinal obstruction, the recovery time of gastrointestinal function and the satisfaction rate of treatment were compared between the two groups. Results The incidence of intestinal obstruction recovery time in the observation group were better than those recovery time of gastrointestinal function and the average in the control group （P〈0.05）. The satisfaction rate of the control group was 81.00%, which was lower than 95.00% of the observation group, the difference was statistically significant （P〈O.05）. Conclusion For abdominal surgery patients, continuous medication of Dachengqi decoction for three months can not only effectively prevent the incidence of adhesive intestinal obstruction, strong safety, simple and practical, effective, but also prevent the reoperation, reduce the economic pressure of the families of patients. It is very suitable for primary hospitals, which is worthy of further promotion in clinic.
