早期大黄辅助联合肠内营养在重症急性胰腺炎治疗中的价值

Early Rhubarb Auxiliary Joint the Value of Enteral Nutrition in Severe Acute Pancreatitis Treatment

目的:探讨早期大黄辅助治疗联合肠内营养在重症急性胰腺炎(SAP)治疗中的临床价值。方法:收集2013年1月-2016年8月间在我院治疗的符合SAP入选标准的51例患者,联合组:早期大黄辅助联合肠内营养(EN)26例,对照组完全胃肠外营养(TNP)25例,比较不同方案两组患者治疗后肠道功能恢复时间、C反应蛋白(CRP)、APACHE-Ⅱ评分、血清白蛋白、平均住院时间、住院费用及并发症的情况。结果:联合组肠道功能恢复时间、平均住院时间明显缩短,住院费用及并发症明显少于TNP组(P<0.05)。治疗10~14d后联合组CRP,APACHE-Ⅱ下降明显早于TNP组(P<0.05)。联合组在治疗后7、14、21d,血清白蛋白水平明显高于TNP组(P<0.05)。结论:早期大黄辅助联合EN治疗SAP能明显减轻炎症反应,促进患者肠道功能恢复,改善患者营养状况,减少并发症,降低住院费用,缩短住院时间。

Objective：to explore the early rhubarb auxiliary treatment combined enteral nutrition in the treatment of severe acute pancreatitis （SAP） clinical value.Methods：Collected in January 2013 to August 2016 in our hospital with SAP,the criteria for the 51 patients, joint groups： the early rhubarb auxiliary joint enteral nutrition （EN）,26 cases in the control group completely parenteral nutrition （TNP） in 25 cases,comparing different schemes in the two groups after treatment in patients with intestinal function recovery time,C-reactive protein （CRP） and APACHEⅡscore, serum albumin, the average length of hospital stay,complications and hospitalization expenses.Results：The joint group of intestinal function recovery time,the average hospitalization time significantly shortened,hospitalization costs and complications significantly fewer than （TNP） group （P〈0.05）.10-14 days after treatment group of CRP,APACHEⅡdecline obviously earlier than （TNP） group （P〈0.05）.Joint group 7,14,21 days after treatment,the serum albumin level was significantly higher （TNP） group （P〈0.05）.Conclusion：Early rhubarb auxiliary joint EN to treat SAP can obviously reduce the inflammatory response, promote the intestinal function recovery,improve the nutritive condition of patients and reduce complications,reduce hospitalization cost,shorten the length of hospital stay.
