经直肠超声引导下挖125I放射性粒子近距离治疗中晚期前列腺癌临床研究

Clinical research of brachytherapy of transrectai ultrasound guided 125I radioactive particles in the treatment of intermediate and advanced prostate carcinoma

目的探讨经直肠超声引导下125I放射性粒子近距离治疗中晚期前列腺癌的临床应用价值及其操作技术，为中晚期前列腺癌的近距离放疗途径提供新思路和新方法。方法将该院2013年1月至2015年11月收治的77例中晚期前列腺癌患者分研究组（39例）和对照组（38例）。研究组先采用经直肠超声引导下125I放射性粒子近距离放疗；对照组予以行双侧睾丸切除术及口服比卡鲁安。结果研究组患者中术后17个月死亡1例，术后14个月出现全身骨转移1例，术后出现PSA生化复发3例。对照组患者中术后12个月死亡2例，术后12个月出现全身骨转移4例，术后出现PSA生化复发7例。结论采用经直肠超声引导下125I放射性粒子近距离治疗局部晚期前列腺癌值得临床推广应用。

Objective To study the clinical value and operation technology of brachytherapy of transrectal ultrasound guided 125I radioactive particles in the treatment of intermediate and advanced prostate carcinoma, and provide new ideas and methods for brachytherapy approach of intermediate and advanced prostate carcinoma. Methods 77 cases of middle and late stage prostate canc- er treated in our hospital from Jan 2013 to Nov 2015 were derided into research group （39 cases） and control group （38 cases）. Pa- tietns in research group were treated with transrectal ultrasound guided 125 I radioactive particles brachytherapy; and patients in control group were treated with bilateral orchiectomy and oral than salomon kalou. Results in the experimental group,there were 1 case of death 17 months after the surgery, 1 case of whole body bone metastases 14 months after the surgery, and 3 cases of PSA bi- ochemical recurrence after surgery. In the control group,there were 2 cases of death 12 months after the surgery, 4 cases of whole body bone metastases 12 months after the surgery,7 cases of PSA biochemical recurrence after surgery. Conclusion transrectal ul- trasound guided 125 I radioactive particles close treatment is worthy of clinical promotion for locally advanced prostate cancer.
