摘要
目的探讨监测血清超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)在脑出血昏迷患者中的动态变化及临床意义。方法选择我院重症监护室收治的42例自发性脑出血昏迷患者为研究对象,并选择30例健康者纳入对照组,比较两组研究对象血清hs-CRP、TNF-α的差异;根据改良MRS量表评分将患者分为预后良好组(n=24)及预后不良组(n=18),监测并比较两组患者血清hs-CRP、TNF-α的动态变化。结果脑出血组患者血清hs-CRP及TNF-α水平均明显高于对照组,差异均有统计学意义(P<0.05);预后良好组与预后不良组患者血清hs-CRP及TNF-α均在发病3d时达到高峰,发病7d、14d时逐步下降,差异均有统计学意义(P<0.05);预后不良组患者在不同时间点血清hs-CRP及TNF-α均明显高于预后良好组,差异均有统计学意义(P<0.05)。结论脑出血昏迷患者血清hs-CRP及TNF-α可出现明显增高,随着病程进展血清hs-CRP及TNF-α出现了动态变化,并且血清hs-CRP及TNF-α与患者的预后关系密切。
Objective To investigate the dynamic changes and clinical significance of serum high sensitive C reactive protein (hs-CRP) and Tumor necrosis factor-α(TNF-α)in patients with cerebral hemorrhage coma. Methods 42 cases of spontaneous cerebral hemorrhage coma patients in the ICU of our hospital as the research object, and 30 healthy peo- ple were included into the control group, the difference between the two groups of serum hs-CRP and TNF-α were com- pared; according to the modified MRS scale scores, they were divided into good prognosis group (n= 24) and poor prognosis group (n= 18). Dynamic change of serum hs-CRP and TNF-α were compared in the two groups. Results the serum hs-CRP and TNF-α in cerebral hemorrhage group was significantly higher than that in the control group the difference was of statistically significant (P〈0.05) ; the serum hs-CRP and TNF-α in the good prognosis group and poor prognosis group at 3 d reached the peak,and at 7 d and 14 d gradually decreased gradually,the differences was of statistically significant (P〈0.05) ;the serum hs-CRP and TNF-α in poor prognosis group at different time points were significantly higher than that of the good prognosis group, the differences was of statistically significant (P〈0.05). Conclusion The serum hs-CRP and TNF-α in patients with cerebral hemorrhage coma is significantly increased, with the course of disease progression tith serum hs-CRP and TNF-α dynamic change, and serum hs-CRP and TNF-α is closely related with the prognosis of patients.
