皮肤软组织扩张器在头面颈脉管畸形治疗中的应用

Application of Skin Soft-tissue Expander in Treating Craniofacial Vascular Malformation

目的通过应用皮肤软组织扩张器,增加头颈部脉管畸形患者病变临近部位的皮肤软组织量,探讨其在头面颈部脉管畸形外科治疗中的作用和优势。方法回顾性分析2013年3月至2015年3月四川大学华西医院烧伤整形外科收治的16例头面颈部脉管畸形病例。所有病例均采取Ⅰ期植入扩张器,定期注水;Ⅱ期切除病变,扩张皮瓣转移覆盖创面的外科手术方法。结果所有病例随访1~2年,均无复发;转移皮瓣与邻近皮肤色泽、外观均相同,质地柔软,切口区无明显瘢痕增生,患者术后随访满意度良好。结论应用Ⅰ期扩张器植入,Ⅱ期切除病变的治疗方式,在切除病变的同时能明显改善患者外观及功能,是治疗头面颈部脉管畸形的有效方法。

Objective To investigate the effect and advantage of the skin soft-tissue expander by applying it to increase the amount of tissue in the adjacent part of the lesion in the treatment of craniofacial vascular malformation.Methods The patients with craniofacial vascular malformation treated from March of 2013 to March of 2015 in West China Hospital of Sichuan University were selected and analyzed retrospectively.All the patients were treated with the combined treatment of first-stage expander implantation and periodic water injection and second-stage pathologic tissue resection and flap transposition.Results The follow-up time of all the cases ranged from 1year to2 years and no recurrence was found during the follow-up.The color and appearance of the transferred flap were almost the same as those of the adjacent skin,the texture was soft and there was no scar hyperplasia in the incision area.Conclusion The combined treatment of first-stage expander implantation and second-stage pathologic tissue resection can excise the lesion and improve the appearance and function of patients′skin obviously at the same time,so it is an effective way to treat craniofacial vascular malformation.
