纤维支气管镜对小儿气管——支气管异物的诊治分析

Diagnosis and treatment of tracheobronchial foreign bodies in children with fiberoptic bronchoscopy

目的:探讨纤维支气管镜在儿童气管-支气管异物诊断治疗中的价值。方法:对我院收治的确诊或者疑诊气管-支气管异物并行纤维支气管镜检查的25例患儿临床资料进行分析。结果:17例确诊支气管异物患儿中男性占12例(70.58%),女性5例(29.41%);年龄11个月~12岁,3岁以下患儿占所有患儿的82.2%;确诊支气管异物患儿中仅有6例(35.2%)在24h内得到及时诊治,11例(64.7%)于异物吸入24h后才得到诊治;有明确异物史9例;25例患儿入院均行肺CT检查,仅有10例有明确异物影,25例患儿入院后均行纤维支气管镜检查明确异物吸入17例,有16例在纤维支气管镜下成功取出;25例患儿肺泡灌洗液细菌培养结果均提示未见异常。结论:对于儿童气管-支气管异物,使用纤维支气管镜进行诊治,可以提高诊断准确性和治疗有效率,值得推广。

Objective：Objective to evaluate the value of fiberoptic bronchoscopy in the diagnosis and treatment of bronchial foreign body in children.Methods：In our hospital diagnosed or suspected trachea bronchial foreign body parallel clinical data of 25 cases of fiberoptic bronchoscopy were analyzed.Results：In 17 cases,12 cases were male （70.58%）,5 eases were female（29.41%）.The age of 11 months-12 years old,Children under 3 years of age accounted for about 82.2% of all children;n all patients, only 6 cases （35.2%） received timely diagnosis and treatment in 24h,llcases （64.7%） were diagnosed with 24h after inhalation of foreign body;There were 9 cases of clear foreign body history;25 patients were admitted to the hospital with pulmonary CT examination,only 10 cases had clear foreign body shadow;25 eases of children were admitted to the hospital after fiberoptic bronchoscopy examination of foreign body aspiration in 17 cases,there were 16 cases in the successful removal of fiberoptie bronehoseopy;25 cases of alveolar lavage fluid bacterial culture results showed no abnormalities.Conclusion：The use of fiberoptic bronchoscopy in the diagnosis and treatment of traeheobronchial foreign bodies in children can improve the accuracy and efficiency of the treatment.
