摘要
目的:本研究通过病例收集,探讨Ⅰ型心肾综合征的中医临床证型,为阐明现代医学的临床理化指标和中医证型之间的关联性提供依据。方法:将80例Ⅰ型心肾综合征患者作为观察组,选择同期我院28例健康体检者为正常对照组,并进行中医证型归纳分组,统计分析比较各组间临床理化指标的差异,论证Ⅰ型心肾综合征的中医证型分布调查及与N末端B型利钠肽原相关性。结果:观察组血清中NT-proBNP水平明显高于对照组(P<0.05),且随着病情加深,NT-proBNP的值均也相应递增。Ⅰ型心肾综合征组的阳虚证、气虚证、阴阳两虚证的人数与正常组相比较均具有显著性差异(P<0.05)。结论:NT-proBNP在机体中的水平与Ⅰ型心肾综合征的恶化程度、预后存在相关性,心肾综合征中阳虚证比例最高,并且根据患者血清NT-proBNP水平的变化有助于判断Ⅰ型心肾综合证组的阳虚征、气虚证、阴阳两虚证。
Objective:This study by collecting cases, to explore the TCM Syndromes of type Ⅰ cardiorenal syndrome, provide the basis for the association between modern medicine clinical elucidate the physicochemical indexes and TCM syndrome. Methods: 80 cases of patients with type Ⅰcardiorenal syndrome as the observation group, select the same period in our hospital 28 cases of healthy persons as the normal control group, and TCM syndrome type induction group, statistical analysis of the differences between groups were compared clinical physicochemical indexes, demonstration investigation of TCM Syndromes of type Ⅰ cardiorenal syndrome distribution and N terminal B type natriuretic peptide. Results: The correlation between serum BNT-proBNP levels were significantly higher than that of the control group(P〈0.05), and as the illness deepens, BNT-proBNP values were also increased accordingly. The patients with Yang deficiency syndrome, heart qi deficiency syndrome renal syndrome, number two of yin and yang deficiency syndrome compared with the normal group there were significant differences(P〈0.05). Conclusion: NT-proBNP in the machine the level of body and type Ⅰcardiorenal syndrome degree of deterioration, the correlation between the prognosis of type Ⅰ cardiorenal syndrome of Yang deficiency syndrome in the highest proportion, and according to the serum NT-proBNP levels are helpful to evaluate the cardiorenal syndrome group of Yang deficiency syndrome, Qi deficiency syndrome, yin and yang deficiency syndrome in two.
