围术期保温干预对腹腔镜胃癌根治术后患者Th1/Th2平衡的影响

Effect of Heat Preservation Intervention During the Perioperative Period on the Th1/Th2 Balance of Patients after the Laparoscopic Radical Gastrectomy

目的探讨围术期保温干预对腹腔镜胃癌根治术后患者Th1/Th2平衡的影响。方法该院择期腹腔镜胃癌根治术患者80例(2014年4月—2015年5月),采用随机数字表法分为2组(n=40):主动保温组(A组)和对照组(C组)。分别于术前1 h(T0)、手术结束时(T1)和术后1、3、7 d(T_(2~4))采集患者外周静脉血,采用ELISA法测定血清Th1型细胞水平(IFN-γ)和Th2型细胞水平(IL-4),计算Th1/Th2比值。结果 A组T1~3时Th1型细胞因子(IFN-γ)分别为(273±29)、(258±30)、(270±33)pg/mL,与C组比较明显升高(P<0.05);A组T1~4时Th1/Th2比值分别为(2.085±0.133)、(1.990±0.211)、(1.914±0.150)、(2.386±0.310),与C组比较明显升高(P<0.05)。结论围术期保温干预对腹腔镜胃癌根治术后患者Th1型细胞因子具有一定的保护作用,有利于维护机体细胞免疫功能。

Objective To study the effect of heat preservation intervention during the perioperative period on the Th1/Th2 balance of patients after the laparoscopic radical gastrectomy.Methods 80 cases of patients with selective laparoscopic radical gastrectomy from April 2014 to May 2015 were selected and randomly divided into the active heat preservation group（group A） and the control group,（group C） with 40 cases in each,and the peripheral venous blood was collected at 1h before operation（T0）,at the end of operation（T1）and at 1,3,7 d after operation（T（2～4））,and the IFN-γ and IL-4 were measured by the ELISA method and the ratio of Th1/Th2 was calculated.Results The IFN-γ levels at T（1～3） in the group A was respectively（273±29）,[（258±30）,（270±33）pg/mL,which were obviously increased compared with those in the group C（P〈0.05）,and the ratio of Th1/Th2 at T（1～4） was respectively[（2.085±0.133）,（1.990±0.211）,（1.914±0.150）,（2.386±0.310）],which was obviously increased compared with that in the group C（P〈0.05）.Conclusion The heat preservation intervention during the perioperative period has a certain protection effect on the IFN-γ of patients after the laparoscopic radical gastrectomy,which is conducive to maintaining the organ cell immune function.
