摘要 目的探讨乳腺良性肿瘤、恶性肿瘤和自身免疫性甲状腺疾病两者之间存在的相关性。方法通过整群收集了新疆维吾尔自治区肿瘤医院2016年1—6月中收治的268例的乳腺良、恶性肿瘤患者的检查资料,通过检测血清甲状腺激素水平(血清TSH、TT3、TT4、FT3、FT4)的检测结果等指标来研究患有乳腺癌的患者在甲状腺功能上是否存在异常改变,及其与乳腺癌疾病的临床病理特征之间的联系。通过将患有乳腺疾病的患者分为乳腺癌组和乳腺良性肿瘤组两组。通过化学发光法检测法来检测TT3、TT4、FT3、FT4、TSH、TPO-Ab水平,同时进行彩超准备观察甲状腺是否出现异常,通过统计学t检验方法来分析乳腺良、恶性肿瘤与自身性免疫性甲状腺疾病的相关性。结果乳腺癌组血清FT4水平(20.15±19.78)pmol/L,高于良性组血清FT4水平(16.58±4.81)pmol/L。乳腺癌组血清FT3水平(4.55±0.60)pmol/L,基本持平于良性组血清FT3水平(4.74±1.51)pmol/L。乳腺癌组TT3水平(1.76±0.32)pmol/L,基本持平于良性组TT3水平(1.76±0.36)pmol/L。FT3和TSH水平的差异无统计学意义(P=0.145,0.178,P>0.05)FT4的水平差异有统计学意义(P=0.03)。结论血清FT4这个血清指标说明了乳腺癌患者和甲状腺疾病的存在有一定的相关性,TPOAb表达、PR表达以及患病率与乳腺癌患者也有着明显的影响,乳腺良、恶性肿瘤与自身性免疫性甲状腺疾病拥有一定的相关性。 Objective To study the correlation between the Benign and malignant breast tumors and autoimmune thyroid disease. Methods 268 cases of patients with benign and malignant breast tumors admitted and treated in our hospital from January to June 2016 were group selected and the serum TSH,TT3,TT4,FT3, FT4 levels were tested to research whether breast cancer patients had the abnormal changes in the thyroid function and the correlation with the clinical pathological features of breast cancer, and the patients with breast disease were divided into the breast cancer group and benign breast tumor group, and the TT3,TT4,FT3,FT4,TSH,TPO-Ab levels were tested by the chemoluminescence, and the whether the thyroid was abnormal or not was observed by the color ultrasound and the statistical t test was used to analyze the correlation between the benign and malignant breast tumors and autoimmune thyroid disease. Results The serum FT4 level in the breast cancer was higher than that in the benign group[(20.15±19.78)pmol/L vs(16.58±4.81)pmol/L], the serum FT3 level in the breast cancer group was basically consistent with that in the benign group [(4.55±0.60)pmol/L vs(4.74±1.51)pmol/L], the TT3 level in the breast cancer group was basically consistent with that in the benign group, [(1.76±0.32)pmol/L vs(1.76±0.36)pmol/L], and there were no obvious differences in the FT3 and TSH values, and there was an obvious difference in the FT4 level(P=0.145,0.178,0.03), and the differences in the FT3 and TSH levels were not statistically significant(P〈0.05). Conclusion The serum FT4 shows that there is a certain correlation between the breast cancer and thyroid disease.And TPOAb expression, PR expression and morality have an obvious effect on the patients with breast cancer, and the benign and malignant breast tumors and autoimmune thyroid disease have a certain correlation.
