通络补心化瘀汤联合八段锦和运动康复训练对冠心病合并急性心力衰竭病人心率恢复和运动功能的影响

Effects of Baduanjin,Exercise Rehabilitation Training and Tongluo Buxin Huayu Decoction on Heart Rate Recovery and Motor Function in Patients with Coronary Heart Disease Complicated with Acute Heart Failure

目的观察通络补心化瘀汤联合八段锦和运动康复训练对冠心病合并急性心力衰竭病人心率恢复和运动功能的影响。方法选取西安医学院附属医院2015年3月—2016年3月心内科住院治疗的62例冠心病合并急性心力衰竭病人,按照随机数字表法分为对照组和观察组,各31例。对照组单纯采用西医常规治疗,观察组在对照组基础上采用通络补心化瘀汤联合八段锦和运动康复治疗,观察两组病人临床疗效、治疗前后血脂水平变化、心率恢复情况、心脏彩超测定值、运动功能及不良反应发生情况。结果观察组临床总有效率明显高于对照组,两组比较差异有统计学意义(P<0.05);治疗后,两组血脂水平、各心功能参数及运动功能水平均优于治疗前,且观察组更优(P<0.05);治疗后,观察组病人心率呈逐渐减少趋势,并趋于稳定,对照组无明显改善(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论通络补心化瘀汤联合八段锦和运动康复训练治疗冠心病合并急性心力衰竭病人疗效确切,能明显改善病人心功能水平,促进心率和运动功能恢复,不良反应少,是安全有效的。

Objective To observe the effects of Baduanjin,exercise rehabilitation training and Tongluo Buxin Huayu decoction（TBHD）on heart rate recovery and motor function in patients with coronary heart disease（CHD）complicated with acute heart failure（AHF）.Methods Sixty-two patients with CHD complicated with AHF from March 2015 to March 2016 were randomly divided into two groups：control group（n =31）treated with routine treatment,and treatment group（n =31）treated with routine treatment and TBHD,Baduanjin,exercise rehabilitation training.The clinical efficacy,changes of blood lipid level,heart rate recovery,heart color Doppler ultrasonography,motor function and adverse reaction were observed before and after treatment.Results The clinical efficacy in treatment group was obviously higher than that in control group（P〈 0.05）.After treatment,the levels of blood lipid and the cardiac function parameters and the movement function were improved in two groups,which was better in in treatment group than that in control group（P 〈0.05）.The heart rate showed a decreasing trend,and tended to be stable in treatment group.There was no significant difference in the incidence of adverse reactions between two groups（P 〉0.05）.Conclusion TBHD,Baduanjin and exercise rehabilitation training can improve cardiac function,promote the heart rate and motor function recovery in patients with CHD complicated with AHF.
