透明质酸预防腹部手术后瘢痕的临床效果观察

Clinical observation of hyaluronic acid in preventing scar after abdominal surgery

目的观察透明质酸在腹部手术后预防其瘢痕形成的临床效果。方法前瞻性选取本院2013年2月~2017年2月经腹部外科手术患者90例作为研究对象,采用信封法随机分为观察组和对照组,各45例,观察组在外科手术关腹后于切口周围注射透明质酸皮肤保湿因子,再进行皮肤缝合;而对照组则直接行常规皮肤缝合。记录两组患者术后2周、1月、3月、6月其瘢痕形成情况及患者自觉痒痛情况。结果两组患者术后2周瘢痕宽度及高度、VAS评分、ODI评分比较差异无统计学意义;术后1个月、3个月、6个月其瘢痕宽度及高度均大于术后2周(P<0.05);但观察组其瘢痕变化程度小于对照组,其差异比较有统计学意义(P<0.05)。两组患者随访各时段VAS评分、ODI评分均呈下降趋势且术后1个月、3个月、6个月与术后2周比较差异有统计学意义(P<0.05),而观察组各时段VAS评分、ODI评分改善程度均较对照组明显,比较其差异有统计学意义(P<0.05)。结论将透明质酸(HA)分期注射于患者腹部外科手术切口周围,能够有效减少瘢痕、抑制瘢痕进一步增生,同时也明显减轻患者术后与瘢痕相关的不适自觉症状,改善患者术后生活质量。值得临床推广运用。

Objective To observe the clinical effect of hyaluronic acid in preventing scar formation after abdominal surgery.Methods 90 patients with abdominal surgery from February 2013 to 2017 were selected as the subjects.The enveloping method was randomLy divided into observation group and control group.There were 45 cases in each group.In the observation group,Abdomen after the incision around the injection of hyaluronic acid skin moisturizing factor,and then the skin suture;and the control group was routine routine skin suture.The scar formation and the spontaneous itching were observed in 2 weeks,1 month,3 months and 6 months after operation.Results There was no significant difference in the width and height of the scar between the two groups.The width and height of the scar were significantly higher than those of the two groups.However,the degree of scarring in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).There was a significant difference between the two groups.The VAS and ODI scores of the two groups were significantly lower than those of the two groups(P<0.05) The improvement of VAS score and ODI score was significantly higher than that of the control group(P<0.05).Conclusion Hyaluronic acid(HA) is injected into the abdominal incision of the patients.It can effectively reduce the scar and inhibit the further proliferation of the scar.It also reduces the discomfortable symptoms associated with the scar after the operation and improves the quality of life The Worthy of clinical use.
