单侧甲状腺腺叶切除联合对侧肿物切除或大部切除术与双侧甲状腺大部切除术治疗双侧结节性甲状腺肿的效果比较

Comparison on the effects of unilateral thyroid lobectomy combined with contralateral subtotal resection or subtotal resection and bilateral subtotal thyroidectomy in the treatment of patients with bilateral nodular goiter

目的:探讨双侧结节性甲状腺肿患者运用双侧甲状腺大部切除术与单侧甲状腺腺叶切除联合对侧肿物切除或大部切除术治疗的临床效果。方法:收治双侧结节性甲状腺肿患者80例,平分两组。对照组采用双侧甲状腺大部切除术治疗,试验组采用单侧甲状腺腺叶切除联合对侧肿物切除或大部切除术治疗,比较两组复发率与并发症发生率。结果:试验组并发症发生率5%,复发率2.5%,明显低于对照组,差异具有统计学意义(P<0.05)。结论:单侧甲状腺腺叶切除联合对侧肿物切除或大部切除术治疗双侧结节性甲状腺肿效果确切,并发症较少,复发率较低。

Objective:To investigate the effects of unilateral thyroid lobectomy combined with contralateral subtotal resection or subtotal resection and bilateral subtotal thyroidectomy in the treatment of patients with bilateral nodular goiter.Methods:80patients with bilateral nodular goiter were selected.They were randomly divided into the two groups on average.The control group was treated with bilateral subtotal thyroidectomy.The experimental group was treated with unilateral thyroid lobectomy combined with contralateral tumor resection or subtotal resection.The recurrence rate and complication rate were compared between groups.Results:The complication rate of the experimental group was 5%,and the recurrence rate was 2.5%,which were significantly lower than those of the control group,with statistically significant difference(P<0.05).Conclusion:Unilateral thyroid lobectomy combined with contralateral tumor resection or subtotal resection in the treatment of bilateral nodular goiter is effective,with fewer complications and lower recurrence rate.
