超声造影对下肢深静脉血栓的诊断价值研究

Evaluating the diagnostic value of contrast enhanced sonography to suspected lower extremity deep venous thrombosis

目的探讨超声造影对下肢深静脉血栓的诊断价值。方法对25例怀疑左侧下肢深静脉血栓形成(DVT)患者在进行常规超声检查后,再进行外周静脉超声造影,随后从腘静脉插管进行顺行静脉造影,比较分析常规超声、超声造影和顺行静脉造影的诊断结果。结果常规超声诊断22例患者下肢DVT,3例不能明确诊断;超声造影明确诊断24例患者DVT,1例为深静脉高凝状态;静脉造影诊断24例DVT,1例无静脉血栓。结论超声造影可替代静脉造影对下肢深静脉是否血栓形成进行准确诊断。

Objective To determine the potential of real-time contrast-enhanced ultrasound imaging in the characterization of lower extremity deep venous thrombosis(LEDVT).Methods A prospective systematic evaluation of routine ultrasonic examination and real-time contrast-enhanced ultrasound examination was taken by an experienced sonographers in patients suspected LEDVT.Then anterograde venography was conducted.The final diagnosis of LEDVT was based on anterograde venography analysis through popliteal vein.Then,these diagnostic results with the results of contrast-enhanced ultrasound and conventional ultrasound examination were compared.Results A total of 25 suspected LEDVT patients were enrolled in our study.Anterograde venography showed 24 patients were finally diagnosed with LEDVT,and 1 case was diagnosed negative with LEDVT.The routine ultrasonic examination was positive in 22 cases(20 actually LEDVT positive),indeterminate in 3 cases.Contrast-enhanced sonography had 100% sensitivity and specificity for identifying LEDVT,showing the same results with anterograde venography.Conclusion Our results show that contrast-enhanced ultrasound imaging can be a useful imaging technique for accurately diagnosing suspected LEDVT.
