摘要
目的探讨幽门螺杆菌(Hp)感染对免疫性血小板减少症(ITP)患儿免疫水平的影响。方法选择2014年6月-2016年6月医院儿科收治的免疫性血小板减少症的患儿72例,作为ITP组,其中ITP组又分为Hp阳性34例,阴性38例;同时选取同一时期在医院行健康检查的儿童44例,作为对照组;比较对照组和ITP患儿组的Hp感染率;比较ITP患儿组与对照组及ITP组中Hp阳性亚组与Hp阴性亚组淋巴细胞亚群表达情况;比较ITP组与对照组及ITP组中Hp阳性亚组与Hp阴性亚组血清中免疫因子的表达情况。结果 ITP患儿感染Hp的感染率显著高于对照组儿童(P<0.05);ITP患儿的免疫细胞表达水平较对照组儿童显著改变,具有显著性差异(P<0.05),但Hp阳性患儿的免疫细胞表达水平较Hp阴性患儿未发生显著性变化;ITP患儿的免疫因子表达较对照组儿童发生显著性变化,Hp阳性患儿的免疫因子表达水平较Hp阴性患儿发生显著性变化。结论ITP患儿存在Hp感染高发的现象且存在免疫细胞表达水平的明显改变,其中Hp阳性患儿免疫因子表达水平也发生显著变化,值得临床治疗重视。
OBJECTIVE To explore the influence of Helicobacter pylori infection on immune level of children with immune thrombocytopenia (ITP).METHODS A total of 72 children with immune thrombocytopenia who were treated in department of pediatrics from Jun 2014 to Jun 2016 were enrolled in the study and assigned as the ITP group,and the children in the ITP group were divided into the Hp-positive group with 34 cases and the Hp-negarive group with 38 cases;meanwhile,44 healthy children who received physical examination during the same period were set as the control group.The incidence rate of H.pylori infection was compared between the control group and the ITP group,the expression of lymphocyte subsets was compared between the ITP group and the control group as well as between the Hp-positive group and the Hp-negative group;the expression of serum immune factors was compared between the ITP group and the control group as well as between the Hp-positive group and the Hp-negative group.RESULTS The incidence rate of H.pylori infection of the ITP group was significantly higher than that of the control group (P〈0.05);the levels of expression of immune cells changed more significantly in the ITP group than in the control group (P〈0.05),however,the levels of expression of immune cells changed more significantly in the Hp-positive children than in the Hp-negative children.The expression of immune factors changed more significantly in the ITP group than in the control group,and the levels of expression of immune factors changed more significantly in the Hp-positive children than in the Hp-negative children.CONCLUSION The H.pylori infection is prevalent among the children with ITP,the levels of expression of immune cells change significantly,and the levels of expression of immune factors also change significantly in the Hppositive children,of which the clinical treatment is worthy to be attached great importance to.
