高血压患者血压变异性和血清Cystatin C的相关性研究

The relationship between blood pressure variability and CystatinC in patients with hypertension

目的探讨高血压患者血压变异性和血清胱抑素C(Cystatin C)的相关性。方法选取高血压患者110例(高血压组),健康对照组120例。所有研究对象检测血清Cystatin C,并行24 h动态血压监测,血压变异性以血压标准差和变异系数表示,比较各组指标,分析血压变异性与Cystatin C的相关性。结果高血压组24 h动态血压参数、血压变异性指标、Cystatin C均高于对照组(P<0.05);直线相关性分析显示,高血压组24 h-SBPSD、24 h-SBPCV、d SBPSD、d SBPCV、d DBPSD、d DBPCV、n SBPSD、n SBPCV与Cystatin C呈正相关性;Cystatin C对肾脏早期损害预测的ROC曲线下面积为0.728,其敏感性71.8%、特异性为70.6%。结论高血压患者血压变异性是早期肾损害的重要预测指标。

Objective To investigate the relationship between blood pressure variability（BPV） and CystatinC in patients with hypertension. Methods 110 patients with hypertension and 120 normal controls were studied. CystatinC and 24 h ambulatory blood pressure levels were detected. Standard deviation（SD） and coefficient of variation（CV） of blood pressure were calculated as the BPV. The relationship between BPV and CystatinC were analyzed. Results The measurement of 24 h ambulatory blood pressure, blood pressure variability and CystatinC in hypertension group were significantly higher than those in control group（ P 〈 0.05 ）. Linear correlation analy- sis showed that the CystatinC was positively correlated with 24 h - SBPSD, 24 h - SBPCV, dSBPSD, dSBPCV, dDBPSD, nSBPSD and nSBPCV. The area under ROC cure of CystatinC was 0. 728. The sensibility was 71.8% and specificity was 70.6%. Conclusion The BPV was better sensitive predictor for early injury in the patients with hypertension.
