2014年-2015年龙岩市非EV71、非CoxA16手足口病肠道病毒病原谱分析

Pathogen spectrum analysis of non-EV71,non-CoxA16 enteroviruses causing hand-foot-mouth disease in Longyan during 2014-2015

目的了解2014年-2015年龙岩市判定为其他EV的HFMD患者病原构成情况,为龙岩市的HFMD防控提供科学依据。方法对580例其他EV HFMD患者的标本用RT-PCR方法检测Cox A6、Cox A10和Echo30,仍无法分型的样本,经RD细胞分离培养后,再次对培养物进行特异性核酸检测。结果 580份样本中检出EV阳性580份,阳性率为100.00%。Cox A6检出率最高,达61.38%,2015年高达71.55%。HFMD其他EV样本分布均以Cox A6为主,阳性检出率高峰均在10月。阳性检出人群主要集中在3岁及以下儿童,占检测总样本数的91.03%,其中2岁~组所占比例最高,为39.48%,不同年龄组阳性检出率差异有统计学意义(χ~2=416.88,P<0.05)。结论 2014年-2015年引起龙岩市HFMD的其他EV中,病原型别众多,尤其是Cox A6已成为本市HFMD的主要病原体,因此建议将其纳入HFMD的日常监测,并在其多发季节加大防控力度。

Objective To understand the pathogenic spectrum of non - EV71, non - CoxA16 enteroviruses causing HFMD in Longyan from 2014 to 2015, so as to provide scientific basis for the prevention and control of HFMD. Methods 580 cases of other EV HFMD specimens were conducted for the detection of CoxA6, CoxA10 and Echo30 by RT - PCR method. Unclassified strains were cultured and isolated by using the human rhabdomyosarcoma（RD） cells, then identified through detection of specif- ic nucleic acid sequences. Results 580 specimens（ 100.00% ） were tested with EV positive by RT - PCR. Among all speci- mens, CoxA6 get the highest detection rate（61.38% ）, reached up to 71.55% in 2015. The peak of positive rate appeared in October, with CoxA6 as the dominant for other EV specimens. Positive population mainly concentrated in the children under 3 years old（91.03% ）, and the 2 - age group had the highest proportion（39.48% ）. The differences of detection rate among different age groups had statistical sigmficance （X= 416.88, P 〈 0.05 ）. Conclusion HFMD in Longyan was concerned with di- verse enterovimses, and CoxA6 was the predominant type among non -EV71, non - CoxA16 enteroviruses. It＇s highly recommended to incorporate CoxA6 into HFMD surveillance system, and scale up control interventions.
