尿酸干预对老年高血压合并糖尿病患者心功能的影响

目的为了了解尿酸干预对高尿酸血症的老年高血压合并糖尿病患者心功能的影响,探讨常规治疗基础上辅以降尿酸治疗的价值和意义。方法按照随机数字表的方法选取100例于2013年6月至2015年5月在我院就诊的伴有高尿酸血症的老年高血压合并糖尿病患者作为研究对象,并依据患者个人的意愿将其分为尿酸干预组和对照组各50例,入组的所有患者均常规采取降压及降糖治疗,而干预组同时指导患者低嘌呤饮食并加用苯溴马隆片口服,比较不同治疗方案对患者代谢指标、收缩压及心功能的影响。结果经过研究发现,干预组患者在不同时间点测定的血尿酸及收缩压和对照组相比明显降低(P<0.05);干预组的LVEF、6 min步行距离和对照组相比明显增高,而血浆脑利钠肽则显著降低(P<0.05);试验组患者的1年后复发率和对照组相比显著降低(P<0.05)。结论伴有高尿酸血症的老年高血压合并糖尿病患者在治疗过程中加用苯溴马隆片,能够明显改善患者的血尿酸和血压水平,提高患者心功能。

Objective： To investigate the influence of uric acid intervention on hyperuricemia in elderly patients with hypertension and diabetes, heart function, and explore the value and significance on the basis of routine treatment with uric acid treatment. Methods： 100 cases accompanied by hyperuricemia in elderly patients with hypertension and diabetes mellitus as the object of study were selected according to the random number table method in May June 2013 to 2015 in our hospital and were divided into the uric acid intervention group and the control group with 50 cases in each group according to the wishes of the individual patient. All patients In the control group in the control group had routinely antihypertensive and hypoglycemic treatment, while the intervention group and guide patients with low purine diet and with benzbromarone tablets, the comparison was done of different treatment schemes on metabolic parameters in patients with, systolic blood pressure and heart function influence. Results： After the research discovery, the determination of serum uric acid and contraction of pressure in the intervention group at different time points decreased obviously compared with the control group, and difference between the two groups had statistical significance （P 〈0.05） ; LVEF in the intervention group, and 6 minutes walk distance were significant- ly higher than those in the and the control group, and plasma brain natriuretic peptide decreased significantly, and the differ- ence between the two groups had statistical significance （P 〈 0.05 ）. The recurrence rate in the patients of the experimental group after 1 year were significantly lower than that in the control group, and the difference between the two groups had statistical significance （P 〈 0.05 ）. Conclusion ： The course of treatment with benzbromarone tablets for elderly patients with hyper- tension and diabetes accompanied by hyperuricemia can significantly improve the blood uric acid and blood pressure levels, improve heart function of patients, and the effect is remarkable, which is worthy of promotion and application.
