胃间质瘤CT表现与病理分级的关系

Relationship between CT findings and pathological grading of gastric stromal tumors

目的探讨胃间质瘤CT表现与病理分级的关系以及CT对胃间质瘤术前分级的评估意义。方法选取2014年1月至2016年3月我院经病理证实的胃间质瘤患者27例,进行腹部CT平扫及双期增强扫描,术后进行Miettinen病理分级,统计分析胃间质瘤CT表现与病理分级的相关性。结果胃间质瘤形态、密度、强化方式与病理分级呈显著相关性(P<0.05);胃间质瘤强化程度与病理分级无显著相关性(P>0.05)。当胃间质瘤形态不规则、密度不均匀、肿瘤呈不均匀强化时病理分级危险度更高。结论胃间质瘤CT征象与危险度病理分级具有显著相关性,对于术前分级的评估具有应用价值。

Objective To investigate the relationship between CT findings and pathological grading of gastric stromal tumors and the significance of CT in preoperative grading of gastric stromal tumors. Method Twenty-seven patients with gastric stromal tumors confirmed by pathology fromJanuary 2014 to March 2016 in our hospital were enrolled in this study. Abdominal CT scan, double phase enhanced scan and postoperative Miettinen pathology classification were performed. Statistical analysis of the relationship between CT manifes-tations and pathological grading were performed. Results The morphology, density and strengthening pattern of gastrointestinal stromal tumor and its pathological stage showed significant correlation （P 〈 0.05）. The strengthening of gastrointestinal stromal tumor was not related with pathological stage （P 〉 0.05）. Irregular tu-mor shape, uneven density and non-uniform strengthening was related with higher pathological grade. Con-clusion The CT signs of gastrointestinal stromal tumor is significantly related to its pathological stage which gives a good preoperative grading and evaluation.
