临床药师对高血压患者个体化用药的药学实践

Pharmaceutical practice of individualized drug used in hypertensive patients by clinical pharmacists

目的探讨精准用药门诊临床药师对高血压患者用药方案调整的方法与效果。方法以就诊于精准用药门诊的2例高血压患者的药物治疗为例,临床药师根据药物的临床疗效和患者的病情变化调整降压药物,拟定最佳药物治疗方案。结果 2例高血压患者临床症状明显改善,血压控制良好。结论药师通过精准用药门诊参与高血压患者的药物调整,为患者制订个体化药学服务,保证了患者用药的安全性和有效性。

Objective To explore the method and effect of clinical pharmacists of accurate medication outpatient in adjusting the medication of patients with hypertension. Methods Taking two patients with hypertension as an example, clinical pharmacists adjusted the therapeutic regimen based on clinical efficacy and patients＇ condition and optimized the medication for patients. Results The clinical symptoms of two patients with hypertension were improved and the blood pressure was controlled well. Conclusion The pharmacists are involved in the drug adjustment of patients with hypertension through the accurate medication outpatient, so as to provide individualized pharmaceutical care for the patients, and ensure the safety and effectiveness of the medication.
