臀下动脉股后皮支复合组织瓣修复深度臀部褥疮的临床应用

Application of the composite tissue flap of the posterior cutaneous branch of the inferior gluteal artery in deep decubitus repair

目的探讨臀下动脉股后皮支复合组织瓣修复深度臀部褥疮的可行性及临床疗效。方法自2014年1月至2叭7年1月,对12例深度臀部褥疮患者应用臀下动脉股后皮支复合组织瓣进行创面修复,皮肤缺损面积为8 cm×5 cm^17 cm×10 cm。结果 12例患者的皮瓣全部一期成活,术后14 d拆线,切口愈合良好,供区直接缝合关闭;随访10例患者3~18个月,皮瓣色泽正常,皮温正常,局部感觉尚可,外观满意;臀部褥疮无复发。结论应用臀下动脉股后皮支复合组织瓣修复深度臀部褥疮创面,具有血供可靠,术后皮瓣感觉良好,手术操作较简便,皮瓣成活率较高,褥疮复发率低的特点,疗效满意,值得临床推广。

Objective To explore the technical feasibility and clinical efficacy of the composite tissue flap of the posterior cutaneous branch of the inferior gluteal artery in deep decubitus repair.Methods From January 2014 to January 2017,12 cases with deep decubitus were repaired by the composite tissue flap of the posterior cutaneous branch of the inferior gluteal artery.The skin defect area was from 8 cm × 5 cm to 17 cm × 10 cm.Results All flaps survived primarily.Stitches were taken out at 14 days postoperative.The incision healed well and the donor site closed by direct suture.Ten cases were followed up for 3 to 18 months.The color and temperature of the flaps were normal,local sensation was acceptable,and appearance was satisfactory.No recurrence was observed.Conclusion The application of the composite tissue flap of the posterior cutaneous branch of the inferior gluteal artery has a reliable blood supply,good sensation,a simple operation technique,a high survival rate,and a low recurrence rate.The efficacy was satisfactory and can be widely used in clinic.
