摘要
目的观察复方杜仲健骨颗粒对髌股关节炎(PFOA)关节镜清理术预后的影响。方法笔者自2012-08—2014-12行关节镜清理术治疗120例PFOA,按照随机分组的方法 ,60例术后接受复方杜仲健骨颗粒治疗(观察组),60例不联用复方杜仲健骨颗粒(对照组)。分别于术前及术后4周、3、12个月采用Lysholm评分评价治疗效果。结果所有患者获得随访12~20(14.85±2.18)个月。所有患者术后切口均愈合良好,无感染、神经血管损伤等并发症。2组术后症状均获得改善。术后3、12个月观察组Lysholm评分均优于对照组,差异有统计学意义(P<0.05)。术后4周及术后3个月时观察组Lysholm评分较对照组改善最为显著,在疼痛、关节不稳定及关节肿胀症状的细节评分差异有统计学意义(P<0.05)。结论复方杜仲健骨颗粒可改善髌股关节炎关节镜术后功能及症状,疗效满意。
Objective To evaluate the effect of postoperative use of eucommia bark granule combined with arthroscopic debridement in treatment of patellofemoral arthritis. Methods One hundred and twenty patients with moderate patellofemoral arthritis treated with arthroscopic debridement from August 2012 to December 2014 was randomly divided into treatment group and control group (60 patients each group). Both groups were treated with arthroscopic debridement, patellaplasty, peripateUar denervation and lateral retinaculum release while treatment group additionally used eucommia bark granule postoperatively. Treatment effect was evaluated with Lysholm knee function score preoperatively and at 4 weeks,3 months and 6 months postoperatively. Results All patients were followed up at a mean of (14.85
±2.18)months. All patients gained a well incision healed postoperatively with no complications such as infection and neurovascular injury. Both group gained a symptom relief after surgery. Lysholm score in treatment group was better than that in control group at 3, 6, 12 months postoperatively (P 〈0.05). Treatment group had a significant improvement in Lysholm score (P 〈0.05) than control group at 4 weeks and 3 months postoperatively especially in pain, joint instability and swelling between two groups(P 〈0.05). Conclusion Eucommia bark granule could improve clinical function and symptom in patients with patellofemoral arthritis after arthroscopic debridement and gain a satisfied clinical effect.
