急性心肌梗死患者血清脑钠肽动态变化及其与炎性因子的关系

The relationship between the serum brain natriuretic peptide and inflammatory factors in patients with acute myocardial infarction

摘要 目的:探讨急性心肌梗死患者血清脑钠肽(brain natriuretic peptide,BNP)的动态变化,并分析其与血清C反应蛋白(C reactive protein,CRP)、肿瘤坏死因子-α(tumor necrosis factor,TNF-α)的关系。方法:随机选取急性心肌梗死患者90例,根据改良Gensini冠脉狭窄程度评分分为轻-中度组(51例)和重度组(39例),比较两组患者的血清BNP、CRP、TNF-α水平;观察上述指标在治疗后12 h、24 h和48 h的变化;分析BNP与CRP及TNF-α的关系。结果:重度组患者血清BNP、CRP及TNF-α水平均高于轻-中度组(P<0.05);分别比较治疗前、治疗后12 h、治疗后24 h及治疗后48 h的血清BNP、CRP及TNF-α水平,差异均有统计学意义(P<0.05);患者血清BNP与CRP及TNF-α呈正相关(P<0.05)。结论:急性心肌梗死患者血清BNP水平明显升高,并与血清CRP、TNF-α水平及治疗时间有关。 Objective:To explore the dynamic changes of the serum brain natriuretic peptide (BNP) in patients with acute myocardial infarction and to study its relationship with the C reactive protein (CRP) and tumor necrosis factor (TNF - α). Methods : 90 cases of patients with acute myocardial infarction were randomly selected and divided, according to the modified Gensini coronary stenosis degree score, into light and moderate group (51 cases) and severe group (39 cases). Serum BNP, CRP and TNF -α level 12 h, 24 h and 48 h after treatment were observed and compared, and the relationship between serum BNP and CRP and TNF -α were analyzed. Results : Serum BNP, CRP and TNF - ct level in severe group were significantly higher than those in light and moderate group ( P 〈 0.05 ). There were significant differences between serum BNP, CRP and TNF - u level 12 h, 24 h and 48 h after treatment ( P 〈0. 05 ). Serum BNP had positive correlation with CRP and TNF -~ ( P 〈 0.05 ). Conclusion: BNP level improves significantly in the patients with acute myocardial infarction and is related to serum CRP, TNF -α level and treatment time.
