两种气道湿化方式在ICU气切脱机患者中的应用效果研究 被引量:5

摘要 目的探讨气切脱机患者采用改装文丘里温湿化装置与人工鼻两种湿化方式的应用效果。方法 2016年1~9月96例气管切开脱机的患者随机分为两组,对照组采用人工鼻进行气道湿化,试验组采用改装加热湿化装置进行气道湿化;比较两组湿化效果、不良反应的发生率、ICU住院期间总机械通气时间、住院时间、VAP的发生率、脱机成功率。结果试验组湿化良好的比例明显高于对照组,湿化不良的比例明显低于对照组;不良反应中,试验组气切套管痰痂和堵管的比例明显低于对照组;试验组在ICU住院期间总的机械通气时间明显短于对照组,48小时、120小时的脱机成功率明显高于对照组。结论使用改装的文丘里温湿化装置对气切脱机的患者湿化效果良好、不良反应少、脱机成功率高、总的机械通气时间短,是一种值得推荐的湿化方式。 Objectives To compare the effect of modified modified Venturi temperature humidification device and artificial nose in ICU traeheotomy patients with no ventilation. Methods 96 patients with tracheotomy were randomly divided into two groups from January to September in 2016. The control group was treated with artificial nose humidification. The experimental group was treated with modified Venturi temperature humidification device for airway humidification. The incidence of adverse reactions, the total mechanical ventilation time, the length of hospitalization, the incidence of VAP, and the success rate of off-line during ICU hospitaliza- tion were compared between the two groups. Results The proportion of good humidification in the experimental group was signifi- candy higher than that in the control group, and the proportion of bad humidification was significantly lower than that of the control group. In the adverse reaction, the proportion of sputum and obstruction in the experimental group was significantly lower than that in the control group. The total mechanical ventilation time was significantly shorter in the ICU hospital than in the control group, and the success rate of off-line was significantly higher in the 48 hours and 120 hours than in the control group, and the above cases were statistically significant. Conclusion The modified Venturi humidification device is effective for tracheotomy patients with no ventila- tion, and the effect of humidification is good, the adverse reaction is low, the success rate off-line is high, the total mechanical ventilation time is short, it is a kind of recommended way of humidification.
