早期康复护理干预对急性缺血性脑卒中患者的应用效果分析

Application Effect of Early Rehabilitation Nursing Intervention in Patients with Acute Ischemic Stroke

目的分析早期康复护理干预对急性缺血性脑卒中患者的应用效果和生活质量的影响。方法选取2014-03—2015-03间福建医科大学附属第一医院收治的60例急性缺血性脑卒中患者,按随机数字表法分为观察组和对照组,每组30例,观察组行早期康复护理,对照组行常规护理,两组治疗方法相同,比较两组干预前后生活质量评分,以及出院后对两组Fugl Meyer运动评分进行比较。结果生活质量评分包括躯体功能、社会功能、心理功能,干预前两组生活质量评分无明显差异(P>0.05),干预后观察组评分分别为(24.1±1.5)分、(15.4±1.7)分、(15.4±1.5)分,评分均优于对照组的(33.5±2.8)分、(22.4±2.9)分、(20.8±2.2)分,差异有统计学意义(P<0.05)。两组患者出院3个月后进行Fugl Meyer运动评分,观察组无Ⅰ级患者,Ⅱ级有1例,Ⅲ级1例,Ⅳ级28例;对照组Ⅰ级有11例,Ⅱ级有7例,Ⅲ级3例,Ⅳ级9例,观察组运动障碍程度均低于对照组,差异有统计学意义(P<0.05)。结论早期康复护理干预能有效的改善急性缺血性脑卒中患者肢体运动能力,提高患者生活质量。

Objective To analyze the application effect of early rehabilitation nursing intervention and quality of life of patients with acute ischemic stroke. Methods 60 patients with acute ischemic stroke from March,2014 to March,2015 were selected according to random number table method,and they were divided into observation group and control group,with 30 cases in each group. Observation group received early rehabilitation nursing,and the control group received routine nursing treatment. The quality of life scores and Fugl Meyer sports scores before and after the intervention of two groups were compared. Results The quality of life scores includes physical function,social function and psychological function. No obvious difference was found between two group in the quality of life scores before intervention（ P 〉 0. 05）. Scores after intervention in observation group are（ 24. 1 ± 1. 5）,（ 15. 4 ±1. 7） and（ 15. 4 ± 1. 5）. The scores are better than those of control group（ 33. 5 ± 2. 8）,（ 22. 4 ± 2. 9） and（ 20. 8 ± 2. 2）. There are statistically significant difference（ P 〈 0. 05）. Fugl Meyer sports scores of two groups are observed. In the observation group,there are no patient with Ⅰ level,1 patient with Ⅱ level,1 patient with Ⅲ level and 28 patients with level Ⅳ. In control group,there are 11 patients with Ⅰ level,7 patients with Ⅱ level,3 patients with Ⅲ level and 9 patients with Ⅳ level. Movement disorder degrees of observation group are lower than the control group,and the difference is statistically significant（ P 〈 0. 05）. Conclusion Early rehabilitation nursing intervention can effectively improve the patients with acute ischemic stroke limb motor ability,and improve the patients quality of life.
