乳腺癌患者对病情保密需求及护理对策研究

Confidential Demand of Breast Cancer Patients and Nursing Care Strategies

目的调查分析乳腺癌患者对病情保密需求,以采取相应的护理对策。方法选择2013年6月至2015年12月在我院接受治疗的182例乳腺癌患者,采用自制问卷对乳腺癌患者及其家属进行问卷调查。结果经调查大部分患者知道自己所患疾病及现状,均想了解与疾病有关保健知识及得到丈夫的支持,患者家属在选择告知患者病情的时间时,手术前所占比例最高为81.32%,在选择告知方式方面,家属自己告知所占比例最高为90.66%。79.12%乳腺癌患者认为,将真实病情完全告知患者利大于弊,90.66%认为患者最有权知道自己病情,83.51%认为真实病情应告知患者本人,90.66%的患者十分希望了解自己的真实病情。对患者隐瞒病情会使其感到十分不满、气愤、得不到真正的心理关怀,认为医务人员欺骗了自己,就不再信任他。结论小部分乳腺癌患者对病情有保密需求,对完全告知病情持否定态度,这不仅会造成患者缺乏对疾病现状、保健知识、治疗方案、不良反应的相应了解,还会对医护人员产生不信任,影响治疗。

Objective To investigate the demand for keeping disease condition confidential of breast cancer patients and to explore nursing care strategies. Method 182 breast cancer patients accepting treatment during June 2013 and December 2015 were included, and the patients and their family were surveyed by self-designed questionnaires. Result The results showed that most patients knew about their disease condition and they wanted to know health knowledge about the disease and to gain support from their husbands. When the family told the patients about disease, 81.32% of them chose the time before surgery. 90.66% patients were told by their family.79.12% breast cancer patients thought that telling real diseases to patients had more advantages than disadvantages. 90.66% of them thought that patients had the right to know their own diseases. 83.51% of them thought that real diseases should be told to patients themselves. 90.66% of them were desirable to know their real disease condition. Covering diseases for patients would make them unsatisfied, angry, feel no psychological care and think of being cheated, therefore medical workers might lose trust from patients. Conclusion A small part of breast cancer patients had demand for confidentiality of disease, and held negative opinion towards complete disease informing. Those patients would lack knowledge about disease condition, health care, treatment and side effects, and thus lose trust for medical workers, affecting further treatment.
