自拟活血益气方对气虚血瘀型急性缺血性脑卒中患者临床疗效观察

Clinical Observation on Self-made Huoxue Yiqi Decoction on Blood Stasis and Qi Deficiency Type of Patients with Acute ischemic Stroke

目的观察自拟活血益气方对气虚血瘀型急性缺血性脑卒中患者临床疗效。方法符合标准的急性缺血性脑卒中患者130例,按照随机表法分为观察组与对照组各65例。对照组患者常规治疗基础上口服拜阿司匹林、瑞舒伐他汀治疗,观察组在对照组基础上结合活血益气方。两组疗程均为4周。比较两组治疗前后神经功能缺损程度(NIHSS)评分、蒙特利尔认知评估量表(MoCA)评分、神经元特异性烯醇化酶(NSE)、白介素-6(IL-6)、同型半胱氨酸(Hcy)水平变化,及不良反应情况。结果观察组治疗总有效率高于对照组(P<0.05);两组患者治疗后NIHSS评分和MoCA评分降低(P<0.05);观察组患者治疗后NIHSS评分和MoCA评分低于对照组(P<0.05);两组患者治疗后NIHSS评分和MoCA评分降低(P<0.05);观察组患者治疗后NIHSS评分和MoCA评分低于对照组(P<0.05);两组患者治疗后血清NSE、IL-6、Hcy水平降低(P<0.05);观察组患者治疗后血清NSE、IL-6、Hcy水平低于对照组(P<0.05);两组肝肾功能、血常规、尿常规及心电图均无异常,且无明显不良反应。结论活血益气方对血瘀气虚证急性缺血性脑卒中患者临床疗效显著,具有重要临床研究意义。

Objective To explore the clinical efficacy of Self-made Huoxue Yiqi decoction on the blood stasis due to qi deficiency acute ischemic stroke patients with blood stasis syndrome. Methods 130 patients with acute ischemic stroke were randomly divided into observation group and control group（ n = 65）. Control group was treated with oral administration of atorvastatin,while observation group was treated with the combination of Huoxue Yiqi decoction on the basis of ontrol group. The courses of treatment were 4 weeks in both groups. Compare the two groups on neurological deficit score（ NIHSS）,Montreal cognitive assessment（ MoCA） score,neuron specific enolase（ NSE） and interleukin-6（ IL-6）,homocysteine（ Hcy） levels,and adverse reaction before and after treatment. Results The total effective rate of observation group was 90. 77%,higher than 75. 38% of control group（ P〈0. 05）; the NIHSS score and MoCA score decreased in both groups（ P〈0. 05）,observation group were lower than those of control group（ P〈0. 05）; serum NSE,IL-6 and Hcy levels decreased in both groups（ P〈0. 05）,observation group were lower than those of control group（ P〈0. 05）; blood routine,liver and kidney function,urine routine and electrocardiogram of both groups were normal,and there were no obvious adverse reactions. Conclusion Self-made Huoxue Yiqi decoction is effective on blood stasis and qi deficiency type of acute ischemic stroke patients with important clinical significance.
