经尿道柱状水囊前列腺扩开术治疗高危老年前列腺增生患者的分析

Efficacy of transurethral columnar balloon dilation of prostate in treatment of benign prostatic hyperplasia in high risk elderly patients

目的研究经尿道柱状水囊前列腺扩开术(TUDP)治疗老年良性前列腺增生(BPH)高危患者的临床疗效及安全性。方法入选2015年1月至2016年10月在眉山市中医医院外三科拟实施手术治疗的老年BPH高危患者100例,随机数字表法分为经尿道前列腺等离子双极电切术(BP-TURP)组和TUDP组,每组50例,研究两种术式用于老年BPH高危患者的临床疗效和安全性。结果相比BP-TURP组,TUDP组患者的手术时间、手术出血量、尿管留置时间、术后住院时间缩短,差异有统计学意义(P<0.05)。两组患者治疗前、治疗3个月后的国际前列腺症状评分(IPSS)、残余尿量(RUV)、生活质量评分(QOL)和最大尿流率(Qmax)差异无统计学意义(P>0.05)。两组患者的血尿、尿道狭窄、暂时性尿失禁等并发症的发生差异无统计学意义(P>0.05)。结论 TUDP具有手术创伤小、术后恢复快的优势,可用于老年BPH高危患者。

Objective To investigate the clinical effect and safety of transurethral columnar balloon dilation of prostate（TUDP） in the treatment of benign prostatic hyperplasia （BPH） in high risk elderly patients.Methods Totally 100 elderly patients with high-risk BPH undergoing elective surgical treatment in our hospital from January 2015 to October 2016 were enrolled in this study.They were randomly divided into bipolar plasmakinetic transurethral resection of prostate （BP-TURP）and TUDP group （50 cases for each group）.The clinical efficacy and safety were observed and compared between the 2 groups.Results Compared with the BP-TURP group, the TUDP group had significantly less amount of intraoperative bleeding,and obviously shorter operation time, catheter retention time and postoperative hospital stay length （P〈0.05）.There were no significant differences in international prostate symptom score （IPSS）, residual urine volume （RUV）, quality of life scale （QOL） and maximum urinary flow rate （Qmax） between the 2 groups before and in 3 months after treatment （P〉0.05）.No significant differences were seen in the incidences of complications, such as hematuria, urethral stricture, and temporary urinary incontinence between the 2 groups （P〉0.05）.Conclusion TUDP has the advantages of small incision and quick recovery, and is a good option for high risk elderly BPH patients.
