Expression of TERT in thyroid follicular tumor and its significance

目的探讨端粒酶逆转录酶(telomerase reverse transcriptase,TERT)在甲状腺滤泡性肿瘤中的表达及意义。方法收集甲状腺滤泡性肿瘤病理组织标本54例,滤泡性腺瘤(FA)25例,恶性潜能未定的滤泡性肿瘤(FT-UMP)7例,滤泡癌(FTC)22例,观察TERT在FA、FTC及FT-UMP中的表达情况。结果TERT在FA、FTC及FT-UMP中的阳性表达率分别为44.0%(11/25),100.0%(22/22),100.0%(7/7)。TERT鉴别良恶性甲状腺滤泡性肿瘤时的灵敏度100.0%,特异度56.0%,准确率79.6%,阳性预测值72.5%,阴性预测值100.0%。结论TERT在FTC、FT-UMP中的表达显著高于FA,有助于鉴别甲状腺滤泡性肿瘤的良恶性,为FTC的术前诊断提供新的思路。

Objective To investigate the expression of human telomerase reverse transcriptase（TERT） in thyroid follicular tumors and its significance.Methods The expression of TERT was detected in 25 cases of thyroid follicular adenoma（FA）,7cases of follicular tumor of undetermined malignant potential（FT-UMP） and 22 cases of follicular thyroid carcinoma（FTC）.Results The positive expression of TERT in FA,FTC and FT-UMP groups was 44.0%（11/25）,100.0%（22/22） and 100.0%（7/7）,respectively.The sensitivity,specificity,diagnostic accuracy,positive predictive value and negative predictive value of TERT in the diagnosis of malignant thyroid follicular tumors was 100.0%,56.0%,79.6%,72.5% and 100.0%,respectively.Conclusion The expression of TERT in FTC and FT-UMP is significantly higher than that in FA,indicating that it may differentiate malignant thyroid follicular tumors from benign thyroid tumor.
