潮式关节腔冲洗与玻璃酸钠关节腔内注射联合运动疗法治疗积液型膝骨性关节炎疗效观察

Effect of articular cavity tidal irrigation combined with intraarticular injection of sodium hyaluronate and exercise therapy for treatment of knee osteoarthritis complicated with articular cavity hydrops

目的探讨潮式关节腔冲洗、玻璃酸钠关节腔内注射联合运动疗法治疗积液型膝骨性关节炎(KOA)的临床效果。方法选择2015年1月至2015年12月黄冈市中心医院收治的积液型KOA患者120例,根据治疗方法分为观察组和对照组,每组60例。对照组患者给予关节积液抽吸联合玻璃酸钠关节腔内注射治疗,观察组患者给予潮式关节腔冲洗、玻璃酸钠关节腔内注射联合运动疗法治疗,以视觉模拟评分法(VAS)评定患膝疼痛,以Lysholm膝关节功能评分法评定患膝功能,对2组患者治疗效果进行比较。结果治疗前2组患者膝关节疼痛VAS评分及膝关节功能Lysholm评分比较差异均无统计学意义(P>0.05)。治疗后2组患者膝关节疼痛VAS评分均显著低于治疗前(P<0.05),膝关节功能Lysholm评分均显著高于治疗前(P<0.05)。治疗后,观察组患者膝关节疼痛VAS评分显著低于对照组(P<0.05),膝关节功能Lysholm评分均显著高于对照组(P<0.05)。观察组和对照组患者治疗总有效率分别为90.00%(54/60)、78.33%(47/60),观察组患者治疗总有效率显著高于对照组(χ~2=6.871,P<0.05)。结论潮式关节腔冲洗、玻璃酸钠关节腔内注射联合运动疗法治疗积液型KOA可有效缓解疼痛,改善关节功能。

Objective To investigate the clinical effect of articular cavity tidal irrigation combined with intraarticular injection of sodium hyaluronate and exercise therapy for treatment of knee osteoarthritis（KOA） complicated with articular cavity hydrops.Methods A total of 120 patients with KOA and articular cavity hydrops were selected from January 2015 to December 2015 in Huanggang Central Hospital.The patients were divided into observation group and control group,60 cases in each group.The patients in the control group were treated with intraarticular effusion aspiration combined with intraarticular injection of sodium hyaluronate.The patients in the observation group were treated with articular cavity tidal irrigation,intraarticular injection of sodium hyaluronate and exercise therapy.The pain of the knee was evaluated by visual analogue scale （VAS）,and the knee joint function was evaluated by Lysholm score.The therapeutic effect was compared between the two groups.Results There was no significant difference in VAS score of knee joint pain and Lysholm score of knee joint function between the two groups before treatment （P 〉 0.05）.The VAS score of knee joint pain after treatment was significantly lower than that before treatment in the two groups（P 〈 0.05）.The Lysholm score of knee joint function after treatment was significantly higher than that before treatment in the two groups（P 〈 0.05）.The VAS score of knee joint pain in the observation group was significantly lower than that in the control group （P 〈 0.05）,and the Lysholm score of knee joint fuuction in the observation group was significantly higher than that in the control group after treatment （P 〈 0.05）.The total effective rate in the observation group and the control group was 90.00% （54/60） and 78.33% （47/60） respectively,the total effective rate in the observation group was significantly higher than that in the control group （x2 =6.871,P 〈 0.05）.Conclusion Articular cavity tidal irrigation combined with intraarticular injection of sodium hyaluronate and exercise therapy in the treatment of KOA complicated with articular cavity hydrops can effectively relieve pain and improve joint function.
