胃癌组织趋化因子受体CXCR4的表达及其临床意义

Expression of chemokine receptor CXCR4 in gastric carcinoma and its clinical significance

目的:探讨胃癌组织中趋化因子受体CXCR4的表达与临床病理特征、肿瘤标志物及预后的关系。方法:选择行胃癌根治术的62例胃癌患者,应用免疫组化检测CXCR4在胃癌组织及对应癌旁组织中的表达,分析CXCR4的表达与患者临床病理特性、术前肿瘤标志物及预后间的关系。结果:胃癌组织中CXCR4阳性表达率为82.3%(51/62),明显高于癌旁组织[62.9%(39/62),P=0.016]。胃癌CXCR4表达水平与肿瘤浸润深度,TNM分期,淋巴结转移与否相关(P<0.05),与患者的性别、年龄、肿瘤大小以及分化程度无明显关系(P>0.05)。CXCR4阳性表达与患者术前血清癌胚抗原、CA199值均无明显相关性(P>0.05)。CXCR4高表达患者的生存率低于低表达患者,但差异无统计学意义(P=0.08)。结论:胃癌组织CXCR4的高表达与胃癌的生物学行为有明显关系,与肿瘤发展程度、分期密切相关。

Objective：To investigate the expression of chemokine receptor CXCR4 in gastric carcinoma and evaluate its relationship with clinicpathological features, prognosis and tumor markers.Methods：A total of 62 patients with radical resection of gastric cancer were enrolled.The expression of CXCR4 in gastric carcinoma tissues and paracancerous tissue was detected by immunohistochemical staining.The relationship of the expression of CXCR4 with preoperative tumor markers, the clinicopathological features, postoperative survival were evaluated.Results：The expression of CXCR4 was both detected in gastric cancer tissues and paracancerous tissue, but the positive expression rate of CXCR4 in gastric carcinoma was 82.3%（51/62） that was significantly higher than that in paracancerous tissue[62.9%（39/62）,P=0.016].The expression level of CXCR4 in gastric cancer was correlated with tumor depth, TNM stage and lymph node metastasis （P=0.08）, but had no significant relationship with the sex, age, tumor size and differentiation of patients （P〉0.05）.Survival rate of patients with CXCR4 expression strong was significantly lower than that of patients with weak CXCR4 expression.There was no significant correlation between CXCR4 positive expression and serum carcinoembryonic antigen and CA199 values（P〉0.05）.Conclusion：The high expression of CXCR4 in gastric cancer is significantly related to the biological behavior of gastric cancer and more significantly related to the stage of tumor development and the prognosis.
