高频振荡通气治疗新生儿持续肺动脉高压的研究 被引量:6

摘要 目的研究高频振荡通气治疗新生儿持续肺动脉高压的效果。方法选取我院2015年1月~2017年1月收治的80例新生儿持续肺动脉高压患儿,依据治疗方法将这些患儿分为高频振荡通气组(n=40)和常频机械通气组(n=40)两组,对两组患儿的原发病、血气指标、血压指标、并发症发生情况进行统计分析。结果高频振荡通气组患儿的胎粪吸入性肺炎、吸入性肺炎比例均显著高于常频机械通气组(P<0.05),急性呼吸窘迫综合征、肺炎、湿肺比例均显著低于常频机械通气组(P<0.05),PaO_2显著高于常频机械通气组(P<0.05),FiCO_2、SPAP均显著低于常频机械通气组(P<0.05),并发症发生率5.0%(2/40)显著低于常频机械通气组20.0%(8/40)(P<0.05)。结论高频振荡通气治疗新生儿持续肺动脉高压的效果较常频机械通气好。 Objective To study the effect of high-frequency oscillatory ventilation in the treatment of neonatal continu- ous pulmonary hypertension, nethtods 80 children with neonatal persistent pulmonary hypertension in our hospital from January 2015 to January 2017 were enrolled in this study. These children were divided into high frequency oscillatory ventilation group(n=40) and normal frequency mechanical ventilation group (n=40), The primary disease, blood gas in- dicators, blood pressure indicators and the incidence of complications in the children of two groups were statistically analyzed. Results The proportion of meconium aspiration pneumonia and aspiration pneumonia in the children of high frequency oscillatory ventilation group was significantly higher than that in normal frequency group(P〈0.05). The pro- portion of acute respiratory distress syndrome, pneumonia and wet lung was significantly lower than that of the normal frequency mechanical ventilation(P〈0.05). PaO2 in the high-frequency oscillatory ventilation group was significantly higher than that in normal frequency ventilation group (P〈0.05). The FiCO2 and SPAP in the high-frequency oscillatory ventilation group were significantly lower than those in normal frequency ventilation group(P〈0.05). And the incidence of complications in the high-frequency oscillatory ventilation was 5.0%(2/40), significantly lower than 20.0% in the nor- mal frequency mechanical ventilation group(8/40)(P〈O.05). Conclusion The effect of high frequency oscillatory ventilation is better than that of normal frequency mechanical ventilation on neonatal continuous pulmonary hypertension.
