乳腺癌数字化X线影像特征中分子亚型及病理类型的比较分析

Comparative Analysis of the Molecular and Pathological Subtypes of Breast Cancer with Different Digital Mammography

目的:探讨乳腺癌术前X线影像特征之间的分子亚型及病理类型的差异。方法:收集本院术前行全数字化乳腺成像(full-field digital mammography,FFDM)检查且经手术病理证实为乳腺癌的患者121例作为研究对象,按照乳腺癌的数字化X线影像基本特征、肿块形态及肿块边界情况进行分组,比较分析各组间的分子亚型及病理类型。结果:数字化X线片表现,按照基本特征划分,单纯肿块、单纯钙化及肿块伴钙化乳腺癌患者中,3种分子亚型的比率不完全相同,其中单纯肿块与单纯钙化的三阴型乳腺癌比率比较,差异无统计学意义(P>0.05),其余两两比较,差异均有统计学意义(P<0.05);按照肿块边界划分,清晰、模糊、小分叶及星芒状的乳腺癌患者中,3种分子亚型的比率不完全相同,其中小分叶及星芒的HER-2过表达型比率比较,清晰及模糊的Luminal型、HER-2过表达型的比率比较,差异均无统计学意义(P>0.05),其余两两比较,差异均有统计学意义(P<0.05)。按照肿块边界划分,清晰、模糊、小分叶及星芒状乳腺癌患者中,4种病理类型的比率不完全相同,其中清晰及模糊的其他病理类型比率比较,模糊及小分叶的浸润性小叶癌比率比较,小分叶及星芒的导管内癌比率比较,差异均无统计学意义(P>0.05),其他两两比较,差异均有统计学意义(P<0.05)。结论:乳腺癌数字化X线成像中部分征象与分子亚型及病理类型有关。

Objective：To investigate the differences of molecular and pathological subtypes of breast cancer with different digital mammography features.Method：121 cases of breast cancer confirmed by operation and pathology were selected as the objects of study before the operation of full-field digital mammography（FFDM） examination,according to the basic characteristics,the tumor shape and the boundary condition of the tumor of the digital X-ray images in breast cancer were grouped,the molecular subtypes and pathological types of each group were compared and analyzed.Result：Digital X-ray findings,divided according to the basic features of simple mass,pure calcification and mass with calcification in patients with breast cancer,the ratio of 3 kinds of molecular subtypes were not exactly the same,including simple mass and pure calcification in three negative breast cancer rate comparison,the difference was not statistically significant（P〉0.05）,the rest of the indices between the two groups,the differences were statistically significant（P〈0.05）;in accordance with the mass boundary division,clear,fuzzy,small leaves and astral form breast cancer patients,the ratio of 3 kinds of molecular subtypes were not identical,the HER-2 expression type ratio of small leaves and astral form,clear and fuzzy Luminal type,comparison of ratios of over expression of HER-2,there were no differences statistical significance（P〉0.05）,the rest of the indices between the two groups,the differences were statistically significant（P〈0.05）.In accordance with the mass boundary division,clear,fuzzy,small leaves and astral form patients with breast cancer,the ratios of 4 pathological types were not exactly the same,among other pathological types and the ratio of clear fuzzy comparison,comparison of fuzzy and small leaves ratio of invasive lobular carcinoma,ductal small leaves and comparison astral form in cancer rates,there were no significant differences（P〉0.05）,the rest of the indices between the two groups,the differences were statistically significant（P〈0.05）.Conclusion：Some of the signs of breast cancer are related to the molecular subtypes and pathological types.
