PDCA循环管理在门诊医务人员手卫生依从性规范管理中的应用效果分析

Application Effect Analysion of PDCA Cycle Management in Outpatient Medical Staff Hand Hygiene Compliance Management

目的:研究并探讨PDCA循环管理在门诊医务人员手卫生依从性规范管理中的应用效果。方法:本院门诊自2016年1月起针对门诊医务人员的手卫生依从性状况开展PDCA循环管理,在开展PDCA循环管理后的2016年1-12月阶段内随机抽取75名门诊医务人员作为观察组,另在未开展PDCA循环管理而仅开展常规护理管理的2015年1-12月阶段内随机抽取75名门诊医务人员作为对照组,比较两组手卫生知识知晓情况、手卫生依从性、手卫生规范操作率、手消毒合格率、卫生手消毒细菌菌落数目。结果:观察组医务人员在手清洗指征、手消毒指征、自我防护知识等手卫生知识方面的知晓率均明显高于对照组(P<0.05),而两组医务人员均对手卫生概念较为了解,其手卫生概念知晓率比较,差异无统计学意义(P>0.05)。观察组接触患者前、接触患者且摘除手套后、无菌操作前、液体暴露风险操作后、接触患者周围物品后的手卫生依从性均明显高于对照组(P<0.05)。观察组医务人员的手卫生规范操作率、手消毒合格率均明显高于对照组(P<0.05),且观察组医务人员卫生手消毒后的细菌菌落数目明显少于对照组(P<0.05)。结论:在门诊针对医务人员的手卫生依从性开展PDCA循环管理,可有效规范门诊医务人员的手卫生操作,提高其手卫生依从性,有利于减少医院感染的发生。

Objective：To study and explore application effect of the PDCA cycle management in outpatient medical staff hand hygiene compliance specification management.Method：Since January 2016,PDCA cycle management was implemented for outpatient medical staff hand hygiene compliance status in our hospital clinic,75 outpatient service medical personnel after the PDCA cycle management from January 2016 to December 2016 were randomly selected as the observation group,in addition,75 outpatient medical personnel before the PDCA cycle management from January 2015 to December 2015 were randomly selected as the control group.Hand hygiene knowledge awareness,hand hygiene compliance,hand hygiene standard operating rate,disinfection qualified rate and health hand disinfection number of bacterial colonies of two groups were compared.Result：Awareness rate of hand hygiene knowledge such as hand washing indications,hand disinfection indications and self protection knowledge of the observation group were obviously higher than those of the control group（P〈0.05）,and both two groups understood hand health concept,there was no statistical significance in awareness rate of hand hygiene concept of two groups（P〉0.05）.Hand hygiene compliance of the observation group before contact with patients,during contact with patients and after removing gloves,before aseptic operation,after liquid exposure risk operation,after contact with something around patients were significantly higher than those of the control group（P〈0.05）.Hand hygiene standard operating rate and hand disinfection qualified rate of the observation group were significantly higher than those of the control group（P〈0.05）,and hygiene hand the number of bacterial colonies of the observation group after disinfection was less than that of the control group（P〈0.05）.Conclusion：To carry out the PDCA cycle management in the clinic for hand hygiene compliance of medical staff can effectively regulate the clinic medical staff hand hygiene operation,improve the hand hygiene compliance,it is helpful to reduce the occurrence of hospital infection.
