择期尿路结石手术患者心理应激与治疗依从性的相关性分析

The analysis of correlation between psychological stress and treatment compliance of urinary tract calculi surgical patients

目的研究择期尿路结石手术患者心理应激与治疗依从的相关性及有效的护理方式。方法将我院90例尿路结石手术患者随机分为观察组与对照组,每组45例,两组均实行尿路结石临床护理路径,观察组在临床路径基础上强化心理护理,比较两组护理前后医院应激量表及依从性评分差异,并应用Pearson相关性法分析心理应激与依从性的相关性。结果两组护理后心理应激的各维度评分较护理前均显著降低(P<0.05),且观察组的各维度评分及总分均显著低于对照组(P<0.05)。观察组护理后依从性评分显著高于护理前及对照组护理后(P<0.05),而对照组护理前后差异无统计学意义(P>0.05)。经Pearson相关性分析发现,应激评分与依从性评分呈显著负相关,Pearson相关系数为r=-0.945(P<0.01)。结论心理应激与依从性存在显著的负相关。强化心理护理的尿路手术临床护理路径有助于改善患者围术期不良心理应激,提高治疗依从性,可作为此类患者围术期管理的有效方式。

Objective To analyze the correlation between psychological stress and treatment compliance of urinary tract calculi surgical patients and the nursing methods. Methods 90 patients who were to have operation of urinary tract calculi were randomly divided into the observation group and the control group, with 45 cases in each group. All the patients were offered clinical nursing path, while the observation group were given psychological nursing in addition. The scores reflecting patients＇ psychological stress and treatment compliance were compared between the two groups before and after nursing, and the correlation between psychological stress and treatment compliance were analyzed using Pearson correlation method. Results All the patients＇ psychological stress was statistically lowered in terms of total score and its dimensions after nursing （ P〈0.05 ）, and the score of the observation group was significantly lower than that of the control group （P 〈 0.05 ） . After nursing, the observation group showed significantly higher compliance than before nursing and that of the control group after nursing （P〈0.05）, whereas the control group did not show statistical difference before and after nursing （P 〉 0.05 ） . Through Pearson correlation analysis, patients＇ psychological stress scores were negatively correlated with treatment compliance scores, with （r=-0.945 and P〈0.01 ） . Conclusion Regarding to urinary tract calculi surgical patients, treatment compliance is negatively correlated with their psychological stress. Strengthening psychological nursing on the basis of clinical nursing path is helpful to relieve patients＇ perioperative psychological stress and improve their treatment compliance, and thus is an effective way of perioperative management.
