高龄女性妊娠期高血压疾病对妊娠结局的影响分析

Effect of aged women complicated with gestational hypertension disease on pregnancy outcomes

目的分析高龄产妇并发妊娠期高血压对妊娠结局的影响。方法选取广东省中山市人民医院产科2014年5月至2016年5月收治的并发妊娠期高血压疾病(GHD)的高龄孕产妇40例,同时选择同期年龄<35岁的孕产妇40例GHD作为对照组,对两组产妇的产科并发症发生情况、围产儿结局进行统计分析。结果高龄组子痫前期及子痫、早产发生率均显著高于低龄组(χ~2值分别为7.680、4.114,均P<0.05),两组妊娠高血压心脏病、HELLP综合征、胎盘早剥、产后出血发生率均无显著性差异(χ~2值分别为1.053、0.392、1.053,均P>0.05)。高龄组产妇生长受限发生率显著高于低龄组(χ~2=3.914,P<0.05),两组胎儿窘迫、新生儿窒息、新生儿畸形、围产儿死亡发生率均无显著性差异(χ~2值分别为0.721、1.053,均P>0.05)。结论高龄产妇较低龄产妇并发妊娠期高血压疾病时易发生不良妊娠结局,临床需要重视。

Objective To analyze the effect of aged parturient complicated with gestational hypertension disease（GHD） on pregnancy outcomes. Methods From May 2014 to May 2016,40 aged women complicated with GHD were selected in department of obstetrics in People＇s Hospital of Zhongshan City of Guangdong Province. At the same period,40 cases of GHD under 35 years were selected as control group. Maternal obstetric complications and perinatal outcomes were analyzed for two groups. Results In aged group incidences of preeclampsia,eclampsia and premature birth were significantly higher than those in the control group（χ~2value was 7. 680 and 4. 114,respectively,both P〈0. 05）. There were no significant differences in heart disease,HELLP syndrome,placental abruption,postpartum hemorrhage between two groups（χ~2value was 1. 053,0. 392 and 1. 053,respectively,all P〈0. 05）. Aged group had significantly higher incidence of maternal growth restriction than the control group（χ~2= 3. 914,P〈0. 05）. The differences in the incidence of fetal distress,neonatal asphyxia,birth defects and perinatal deaths were not significant（χ~2value was 0. 721 and 1. 053,respectively,both P〈0. 05）.Conclusion Attention should be paid to poor pregnancy outcomes of aged women complicated with GHD.
