急性下呼吸道感染新生儿及其母亲维生素D水平与疾病关系的研究

Association of maternal vitamin D concentrations and acute lower respiratory infection in newborns

目的探讨急性下呼吸道感染新生儿及其母亲维生素D水平与疾病的相关性。方法选择25例急性下呼吸道感染新生儿及其母亲作为观察组,15例正常新生儿及其母亲作为正常组,ELISA试剂盒检测所有新生儿及其母亲血清25-羟基维生素D[25(OH)D]水平。结果观察组新生儿血清25(OH)D水平为(9.12±1.88)ng/ml,低于正常组新生儿的(16.33±3.42)ng/ml,差异有统计学意义(P<0.05)。观察组母亲血清25(OH)D水平为(13.38±5.96)ng/ml,低于正常组母亲的(22.79±8.79)ng/ml,差异有统计学意义(P<0.05)。观察组和正常组新生儿血清25(OH)D水平与母亲血清25(OH)D水平均呈正相关(r=0.8167和0.9263,均P<0.05)。新生儿血清25(OH)D水平<10ng/ml更容易出现急性下呼吸道感染(OR=4.25,95%CI:1.058~17.070,P=0.041)。结论新生儿血清25(OH)D水平<10ng/ml时即可作为出现急性下呼吸道感染危险的重要指征,产妇补充足量的维生素D可以减少新生儿的维生素D缺乏,进而降低出现急性下呼吸道感染的风险。

Objective To determine the association of maternal serum 25-hydroxyvitamin D [25（OH）D] concentrations and acute lower respiratory infection（ALRI） in newborns. Methods Twenty five newborns with ALRI（study group） admitted to neonatal intensive care unit and 15 healthy newborns（control group） were enrolled in the study. Serum 25（OH）D concentrations in mothers and newborns of both groups were measured by ELISA kit. Results The mean serum 25（OH）D concentrations in newborns of study group were lower than those in control group（9.12±1.88ng/ml vs 16.33±3.42ng/ml, P〈0.05）. Also, mean serum 25（OH）D concentrations in the mothers of study group were lower than those in control group（13.38±5.96ng/ml vs 22.79 ±8.79ng/ml, P〈0.05）. The 25（OH）D concentrations of newborns were highly correlated with maternal serum 25（OH）D concentrations in both groups. Logistic regression showed that 25（OH） D of newborns 10ng/ml was an independent risk factor for ALRI（OR=4.25, 95%CI： 1.058-17.070, P =0.041）. Conclusion Newborns with vitamin D deficiency may have an increased risk of ALRI, the 25（OH）D concentrations in newborns and mothers are strongly correlated, which indicates that adequate vitamin D intake should be emphasized for women during pregnancy.
