HRMRI与MRA评价穿支动脉供血区脑梗死责任血管的价值比较 被引量:1

摘要 目的比较高分辨率磁共振成像(HRMRI)、磁共振血管造影(MRA)两种核磁共振检查手段对穿支动脉供血区脑梗死责任血管的诊断价值。方法选取2014年7月~2016年7月收治的35例穿支动脉供血区脑梗死患者,实施常规MRI及MRA检查,而后按照梗死区解剖学血管支配,实施HRMRI检查,比较HRMRI、MRA识别载体血管狭窄情况、评估狭窄程度的价值。结果与MRA相比,HRMRI测得的载体血管狭窄程度更高,且可显示管径细小的豆纹动脉并更接近实际;HRMRI显示斑块位置及穿支血管能力优于MRA,差异有统计学意义(P<0.05)。以确诊结果为金标准,MRA判断穿支动脉载体血管的准确率为51.52%,低于HRMRI的94.29%,差异有统计学意义(P<0.05);HRMRI判断穿支动脉载体血管狭窄程度的灵敏度为97.06%(33/34)。结论 HRMRI可显示穿支动脉自身管径和开口受累情况,且判断血管狭窄、狭窄程度的价值高于MRA,为穿支动脉供血区脑梗死责任血管的无创评估提供了可靠参考。
