摘要
目的:通过分析甘肃省社会卫生支出状况及其变动趋势,揭示甘肃省社会卫生筹资现状,为提高社会卫生筹资能力提供依据。方法:基于省级卫生总费用来源法核算社会卫生支出,计算增长速度,分析变动趋势。结果:连续20年甘肃省社会卫生支出呈现"W"型筹资特点。1995—2002呈现明显下降趋势,2002年占卫生总费用比重最低仅为18.87%;2003—2005年先升后降。2006年呈缓慢的上升趋势,2014年占卫生总费用比重为28.82%;社会卫生支出构成中社会基本医疗保险和社会办医支出为主,商业保险比重偏低,2014年占社会卫生筹资比重仅9.54%。2014年甘肃省社会卫生支出占卫生总费用比重低于全国平均约10个百分点,人均社会卫生筹资仅为633.69元,低于全国平均水平350元。结论:甘肃省社会卫生筹资能力增长缓慢,筹资能力严重不足,筹资模式和筹资渠道亟待完善优化。
Objective: To analyze the situation and changing trend of social health expenditure in Gansu to reveal the current sta- tus of social health financing and provide the evidence for improving social health financing capacity. Methods: Based on the provin- cial total health expenditure by resource, it calculated social health expenditure, estimate the increasing speed and analyze the chang- ing trend. Results: For 20 consecutive years, the social health expenditure in Gansu Province showed "W" characteristics, which showed a significant decreasing trend from 1995 to 2002. The proportion of total expenditure on health(TilE) accounted for 18,87% in 2002. The trend decreased after increased from 2003 to 2005. There was a slow increasing trend in 2006. The proportion of THE in 2014 was 28.82%. Social basic medical insurance and non-public hospitals took the majority of social health expenditure. The propor- tion of business insurance was respectively low, which took 9.54% of social health financing in 2014. In 2014, the proportion of social health expenditure in THE was 10% lower than the national average level. The social health expenditure per capita was 633.69 yuan, which was lower than the national average level(350 yuan). Conclusion: The financing capacity of social health in Gansu increased slowly, the financing capacity seemed seriously shortage. The financing mode and channels needed to be optimized in short time.
