摘要
目的 :研究下颌骨骨折钛小夹板固定功能负载条件下不同部位骨断端的稳定性。方法 :建立下颌角、下颌体部颏孔区及正中联合区三种单一线性下颌骨骨折体外模型 ,用MNS10 4型钛小夹板及M 2 .0B单皮质螺钉采用不同的方法进行骨折固定。对固定后的下颌骨骨折模型进行体外模拟功能负载。测定下颌功能活动时骨折断端弯曲及扭转运动位移量大小。应用Origin6 .0软件进行结果分析处理。结果 :单一钛小夹板沿中线固定下颌角区、体部颏孔区及正中联合区骨折骨断端均产生较大的弯曲及扭转移位。应用单夹板外斜线固定下颌角骨折扭转移位量降低明显 ,双钛夹板固定体部及正中联合区骨折断端弯曲及扭转移位都显著降低。结论 :单一钛小夹板沿外斜线固定下颌角骨折有利于对抗断端扭转移位 ,双钛夹板平行放置固定体部及正中联合区骨折既能有效地对抗弯曲移位 ,又能有效地对抗扭转移位 。
Objective:This study evaluated the stability of mandibular fractures fixed by titanium miniplates under founctional loading.Methods:Anglem,body and symphyseal single line fracture were simulated in vitro.The fracture was fixed by MNS104 type miniplate and M2.0B unicortex screw.Simulated bite force were applied at 13 bites points.The displacements of the fragments were registered and be coverted into bending and torsion angle.The data were analyzed by Origin6.0 software.Results:A large bending and torsion displacement was founded in angle,body and symphyseal fracture fixed by a single titanium mini-plate along the middle line.While the torsion displacement decreased significantly to angle fracture fixed by one mini-plate alone the external oblique line.To the body and symphyseal fracture there small fragments displacement was founded when treated by two mini-plates.Conclusions:A single mini-plate along external oblique line can neutralize torsion displacement effectively.Two mini-plates can neutralize bending displacement as well as torsion displacement effectively when used to fixed body and symphysial fracture.
