100例糖尿病视网膜病变患者白内障超声乳化术后黄斑水肿的研究

Analysis of macular edema after phacoemulsification surgery for 100 cases of patients with diabetic retinopathy

目的 :探讨合并视网膜病变对糖尿病患者白内障超声乳化术后黄斑水肿发生率的影响。方法 :选取糖尿病视网膜病变患者100例(109眼)作为观察组,选择同期糖尿病非视网膜病变患者100例(107眼)作为对照组。所有患者均进行常规白内障超声乳化吸出联合人工晶状体植入手术;对患者视力、黄斑区视网膜厚度、黄斑水肿的发生率及并发症发生情况进行统计分析。结果 :手术治疗后,对照组89.72%患者视力≥0.5,观察组74.31%患者视力≥0.5(P<0.01);术后1个月,对照组有10例(11眼)发生黄斑水肿,发生率10.28%(11/107),观察组有31例(33眼)发生黄斑水肿,发生率30.28%(33/109),两组比较差异显著;与术前比较,对照组术后1个月及3个月CSMT无显著变化,观察组术后1个月及3个月CSMT均显著增加;观察组术后并发症发生率为11.93%,显著高于对照组的3.74%(P<0.05)。结论 :合并视网膜病变会增加糖尿病患者术后黄斑水肿的发生率。

Objective： To explore the effect of macular edema after phacoemulsification surgery for patients with diabetic retinopathy. Methods： One hundred cases （109 eyes） with diabetic retinopathy were selected as an observation group and meanwhile 100 cases （107 eyes） without diabetic retinopathy as a control group. All the cases underwent phacoemulsification surgery plus intraocular lens implantation and the vision, central macular thickness, the incidence of macular edema and complication were analyzed. Results： After operation, the patients with vision ≥0.5 accounted for 89.72% in the control group and 74.31% in the observation group （P〈0.01）. There were 10 cases （11 eyes） occurred macular edema in the control group and 31 cases in the observation group （10.28% vs 30.28% ） one month after operation. Compared with before operation, there were no significant differences in CSMT at 1st and 3rd month after operation in the control group, however, CSMT was significantly increased in the observation group. The incidence of complications was significant higher in the observation group than in the control group （11.93% vs 3.74%, P〈0.05）. Conclusion： The complicated retinopathy can increase the incidence of postoperative macular edema in diabetic patients.
