血清Cys C和NGAL及U-mAlb/Cr在恶性肿瘤患者化疗肾损害早期诊断中的价值

Value of Serum Cys C,NGAL and U-mAlb/Cr in the Diagnosis of Renal Damage After Chemotherapy

目的探讨血清半胱氨酸蛋白酶抑制剂C(Cys C)和中性粒细胞明胶酶相关脂质运载蛋白(NGAL)及尿微量白蛋白/肌酐比值(U-m Alb/Cr)在恶性肿瘤患者化疗肾损害早期诊断中的价值。方法入组65例恶性肿瘤患者,分别在化疗前、后检测血清Cys C和NGAL及U-m Alb/Cr水平,并以55例健康体检者作为对照,比较各组血清Cys C和NGAL及U-m Alb/Cr。结果恶性肿瘤患者血清Cys C和NGAL及U-m Alb/Cr化疗前分别为(2.28±1.21)mg·L^(-1)、(158.58±20.13)ng·m L^(-1)、(60.12±0.81)mg·g^(-1),化疗后分别为(3.05±1.68)mg·L^(-1)、(237.29±25.67)ng·m L^(-1)、(96.54±12.91)mg·g^(-1);健康体检者分别为(0.84±0.16)mg·L^(-1)、(32.74±2.63)ng·m L^(-1)、(4.15±0.47)mg·g^(-1),恶性肿瘤患者化疗前、后血清Cys C和NGAL及U-m Alb/Cr水平均明显高于健康体检者(P均<0.05),且恶性肿瘤患者化疗后血清Cys C和NGAL及U-m Alb/Cr水平均明显高于化疗前(P均<0.05)。结论血清Cys C和NGAL及U-m Alb/Cr水平对于恶性肿瘤患者化疗肾损害早期发现及治疗具有重要意义。

Objective To investigate the clinical value of detecting the levels of serum cystatin C （Cys C）, neu- trophil gelatinase associated lipocalin （NGAL） and urinary albumin/creatinine ratio （U-mAlb/Cr） in the early di- agnosis of renal damage after chemotherapy. Methods The level of serum Cys C, NGAL and U-mAlb/Cr were de- tected in the 65 patients with malignant tumor before and after chemotherapy and the 55 cases of healthy volunteers. Results The level of serum Cys C, NGAL and U-mAlb/Cr were respectively （ 2. 28 ± 1. 21 ）mg · L^-1, （158.58 ±20.13） ng · mL^-1 , （60. 12 ±0.81） mg· g^-1 before chemotherapy and （3.05 ± 1.68） mg · L^-1 , （ 237.29 ± 25.67 ） ng · mL ^- 1, （96.54 ＋ 12.91 ）mg · g ^-1 after chemotherapy in the cancer patients. Those were （0.84 ± 0. 16） mg · L^-1 , （ 32.74 ± 2.63 ） ng · mL ^- 1, （4. 15 ± 0.47 ） mg · g ^-1in the healthy volunteers, the level of serum Cys C, NGAL and U-mAlb/Cr were significantly higher before and after chemotherapy in the cancer patients than those in the healthy volunteers （ P 〈 0.05 ） ; the level after chemotherapy were higher than those before chemotherapy in the cancer patients （P 〈 0.05）. Conclusion Detection of serum Cys C, NGAL and U-mAlb/Cr had a higher valuation in the early diagnosis and treatment of renal injury after chemotherapy.
