乳腺癌早期诊断及防治因素分析

Early diagnosis of breast cancer and factors influencing the early prevention

目的探讨多层螺旋CT扫描对乳腺癌早期诊断的价值,分析影响乳腺癌早期防治的因素。方法选取2015年3月至2016年4月间上海嘉会国际医院收治的189例术前行多层螺旋CT检查的乳腺癌患者,比较螺旋CT扫描与病理检查诊断结果的符合率。采用单因素分析和多元Logistic回归分析方法,分析乳腺癌早期防治的相关因素。结果螺旋CT扫描与病理检查诊断结果的符合率为93.1%。收入水平、婚姻情况、文化程度、家族史及乳腺疾病史是影响乳腺癌早期防治的因素,差异均有统计学意义(均P<0.01),而年龄、性行为状态、生育次数及吸烟饮酒史与乳腺癌患者早期防治关系不大,差异无统计学意义(P>0.05)。影响乳腺癌早期防治的独立危险因素为乳腺疾病史、家族史、文化程度和收入水平,且危险程度依次降低。结论 CT扫描在乳腺癌的早期诊断中,有较高的准确率,家族史、乳腺疾病史、收入情况与文化程度是影响乳腺癌早期防治的独立危险因素。

Objective To explore the value of multi-slice spiral CT scan in early diagnosis of breast cancer and to analyze the factors influencing the early prevention. Methods The clinical data of 189 patients with breast cancer who received multi-slice spiral CT examination at Shanghai Jiahui International Hospital from March 2015 to April 2016 were collected to determine the coincidence rate between the results of multi-slice spiral CT scan and of pathological diagnosis. Single factor analysis and logistic analysis were used to analyze related factors influencing the early prevention of breast cancer. Results The coincidence rate between the results of multi-slice spiral CT scan and of pathological diagnosis was 93. 1%. Analysis showed that income,marital status,educational level,family history and history of breast disease were related factors influencing the early prevention of breast cancer（all P〈0. 01） rather than age,sex state,history of smoking and drinking（P〉0. 05）. Multiple logistic regression analysis of risk factors for early prevention of breast cancer revealed that history of breast disease,family history,educational level and income were independent risk factors influencing the early prevention of breast cancer,and the degree of risk declined in turn. Conclusion CT scan has high accuracy for the diagnosis of early breast cancer. Family history,marital status,history of breast disease,income and educational level were the independent factors influencing early prevention of breast cancer.
