探讨日间呼吸衰竭和夜间低氧对稳定期慢阻肺病患肺部功能损害程度的影响

目的分析探讨日间呼吸衰竭和夜间低氧对稳定期慢阻肺病患肺部功能损害程度的影响效果。方法选择66例在我院接受治疗的处于稳定期的慢性阻塞性肺疾病患者作为研究对象，了解患者的夜间低氧程度，根据其日问是否出现呼吸衰竭和肺功能严重程度将其进行分组，选择同期年龄相当、肺炎治愈、FEV1／FVC、日间血氧饱和度正常的患者62例作为对照组研究对象，检测不同组患者的日问相关指标，并对日间呼吸衰竭和夜间低氧对患者肺部功能损害程度的影响进行统计学分析。结果66例患者TBgO与FEVl％呈负相关，LSaO2、MSaO2和FEV1％预计值呈负相关；日间PaO2与T1390呈负相关，与MSaO呈正相关。差异均具有统计学意义（P〈0．05）。结果稳定期COPD患者存在夜间低氧的问题，因此在确定患者是否需要接受夜闭氧疗时不能仅根据惑者的肺功能情况决定。可以根据患者日间PaO2的测试结果来进行指导夜间氧疗的进行。

Objective To investigate the daytime and nighttime hypoxic respiratory failure functional extent of damage the lungs in patients with stable chronic obstructive pulmonary disease affect the results. Methods 66 cases in our hospital for treatment of patients with chronic obstructive pulmonary disease in stable phase of the study, to understand the extent of nocturnal hypoxia in patients, ac- cording to their severity during the day if there is respiratory failure and lung function to be grouped Select the same period of considera- ble age, pneumonia cure, FEV1/FVC, normal daytime oxygen saturation as a control group of 62 patients studied, detection of differ- ent groups of patients with daytime relevant indicators, and day and night hypoxic respiratory failure the degree of lung function impair- ment on statistical analysis. Results 66 patients TB90 was negatively correlated with FEV1 %, LSaO2, MSaO2 and FEVI% predicted a negative correlation; daytime PaO2 was negatively correlated with the TB90, was positively correlated with MSaO. The differences were statistically significant （P〈0. 05）. Results The presence of stable COPD patients with nocturnal hypoxia problem, it is not only based on the patient＇s lung function determines, based on daytime PaO2 patient test results to guide nocturnal oxygen therapy in deter- mining whether a patient in need of nocturnal oxygen therapy get on.
