血液滤过治疗肾综合征出血热合并急性肾功能衰竭的临床研究

目的探讨血液滤过治疗肾综合征出血热合并急性肾功能衰竭的临床疗效。方法回顾性分析我院于2014年4月至2015年12月期间，治疗的22例肾综合征出血热合并急性肾功能衰竭患者的临床资料，所有患者在常规治疗的基础上，联合血液滤过治疗；对比患者治疗前后的肾功能改善情况、血清炎症因子消除情况，根据急性生理与慢性健康评分表评分（APACHE II评分），综合评价患者的临床疗效。结果经血液滤过治疗后，患者的血清BUN、SCr、IL-6、IL-10、TNF—α、CRP水平均显著降低，与治疗前对比．具有显著性差异（P〈0．05）；治疗前，患者的APACHE II评分为18．61±3．26分；治疗后，患者的APACHE II评分为10．15±1．53分；治疗前后的APACHE II评分对比，具有显著性差异（P〈0．05）。结论血液滤过治疗肾综合征出血热合并急性肾功能衰竭的临床疗效确切，可保护肾功能，消除血清炎症因子．整体改善患者的预后，临床应用前景广阔。

Objective To investigate the clinical effect of the treatment of hemorrhagic fever with renal syndrome with acute renal failure. Methods A retrospective analysis of our hospital in April 2014 ~ December 2015 period, the treatment of 22 cases of hemor- rhagic fever with renal syndrome with acute renal failure in patients with clinical data, all patients on the basis of routine treatment, combined with hemofiltration therapy, compared before and after treatment in patients with renal function change good situation and se- rum inflammatory factor elimination, according to the acute physiology and chronic health evaluation scale score （APACHE II score）, the comprehensive evaluation of patients with clinical curative effect. Results After hemofiltration treatment, serum BUN, SCR, IL- 6, IL-10, TNF-α, CRP levels were decreased significantly, and the compared with before treatment, with significant difference （P〈 0.05）. Before treatment, the patients of the APACHE II score was 18.61± 3.26 points; after treatment, patients with APACHE II rating is divided into 10.15± 1.53 points, before and after the treatment of the APACHE II score comparison, with significant differ- ence （P〈0.05）. Conclusion The clinical effect of the treatment of hemorrhagic fever with renal syndrome with acute renal failure is accurate, which can protect renal function, eliminate serum inflammatory factors, improve the prognosis of patients, and has broad prospects for clinical application.
