配偶同期系统心理干预对长期血糖控制不良的中青年2型糖尿病性勃起功能障碍患者的影响

The Effect of Spouses′ Concurrent Psychological Intervention on the Young and Middle-aged Patients with Long-term Poor Glycemic Control and Type 2 Diabetic Erectile Dysfunction

目的探讨配偶同期系统心理干预在长期血糖控制不良的中青年2型糖尿病性勃起功能障碍(DED)患者中的应用。方法将2014年1-9月在西安交通大学第一附属医院内分泌科门诊就医的80例DED患者,按随机数字表法分为对照组(n=40)与试验组(n=40)。两组均接受为期6个月的常规心理护理,试验组附加配偶同期系统心理干预。干预前及干预后6个月、1年,比较两组干预前后血糖水平、心理状态及性生活质量。结果干预前两组空腹血糖(FBG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbAlc)水平、焦虑自评量表(SAS)、抑郁自评量表(SDS)、勃起指数(IIEF-5)和性生活质量(SLQQ)评分比较,差异无统计学意义(P>0.05)。干预后6个月、1年,两组FBG、2hPG、HbA1c、SAS、SDS、IIEF-5和SLQQ评分均有改善,且试验组改善明显,差异有统计学意义(P<0.05)。结论对中青年长期血糖控制不良的2型DED患者实施配偶同期系统心理干预,可有效改善患者的血糖水平及心理状态,提高性生活质量。

Objective To explore the effect of spouses′concurrent psychological intervention on the young and middle-aged patients with long-term poor glycemic control and type 2 diabetic erectile dysfunction（DED）.Methods 80 DED patients treated in The First Affiliated Hospital of Xi′an Jiaotong University from January to September in 2014 were selected and divided into the experiment group（n=40）and the control group（n=40）according to the random number table.All the patients of both groups were given the conventional psychological nursing care,while the spouses of the patients in the experiment group were given the concurrent psychological intervention.The comparisons were made between the two groups in terms of the data of the levels of blood glucose,the mental state and the quality of sexual life collected before the psychological intervention,6 months and ayear later after the psychological intervention respectively.Results The levels of fasting blood glucose（FBG）,2-hour postprandial blood glucose（2hPG）and glycosylated hemoglobin（HbAlc）and the scores of Self-rating Anxiety Scale（SAS）,Self-rating Depression Scale（SDS）,International Index of Erectile Function（IIEF-5）and Sexual Life Quality Questionnaire（SLQQ）were not significantly different before the intervention（P 0.05）between the two groups,but they were improved in both groups after the intervention and the improvements of the experiment group were greater than those of the control group（P〈0.05）.Conclusion The spouses′concurrent psychological intervention can improve the levels of blood glucose,the mental state and the quality of sexual life in the young and middle-aged patients with long-term poor glycemic control and type 2diabetic erectile dysfunction.
