经颈静脉肝内门体分流术对老年肝硬化食管胃底静脉曲张破裂出血患者生命质量的影响

The Effect of Transjugular Intrahepatic Portosystemic Shunt on the Life Quality of Elderly Cirrhosis Patients with Esophagogastric Variceal Hemorrhage

目的研究经颈静脉肝内门体分流术(TIPS)对老年肝硬化食管胃底静脉曲张破裂出血患者生命质量的影响,以期为临床实践提供依据。方法选取2013年3月至2014年1月西南医科大学附属医院收治的老年肝硬化食管胃底静脉曲张破裂出血患者97例,采用简明生活质量测定量表(SF-36)分别在入院时、TIPS后6周和TIPS后1年对患者进行生命质量评价,并比较评价结果。结果患者生命质量各维度评分在TIPS后6周有所增加,部分维度略有下降,但在治疗后1年再次进行评价时均有较大程度改善;治疗前后差异均有统计学意义(P<0.05)。结论通过TIPS治疗,患者生命质量得到改善,进一步证实TIPS在肝硬化食管胃底静脉曲张破裂出血患者治疗中的优势。

Objective To explore the effect of transjugular intrahepatic portosystemic shunt（TIPS）on the life quality of elderly cirrhosis patients with esophagogastric variceal hemorrhage and provide the theoretical basis for clinical practices.Methods 97 elderly cirrhosis patients with esophagogastric variceal hemorrhage,who were treated in The Affiliated Hospital of Southwest Medical University between March of 2013 and January of 2014,were selected as research subjects.Their life quality was evaluated at the time of admission,6 weeks and 1 year after the treatment of TIPS respectively by the 36-Item Short-Form Health Survey（SF-36）and the evaluation results were then compared.Results The scores of all the dimensions of the patients′life quality increased after the treatment of TIPS,but the scores 6 weeks after the treatment of TIPS were relatively low and some of them even decreased slightly.However,the scores 1 year after the treatment of TIPS increased to a great degree,and there were significant differences between before and after the treatment of TIPS（P〈0.05）.Conclusion The treatment of TIPS can improve the life quality of elderly cirrhosis patients with esophagogastric variceal hemorrhage,which further confirms the advantages of TIPS.
