复方苦参注射液在晚期胃癌患者辅助化疗中的作用

Effect of compound Kushen injection in adjuvant chemotherapy for patients with advanced gastric cancer

目的:探讨复方苦参注射液在晚期胃癌患者辅助化疗中的作用。方法:将83例晚期胃癌患者按照随机数字表法分为联合组42例和对照组41例,均接受改良FOLFOX6化疗方案,联合组在此基础上加用复方苦参注射液。比较两组治疗效果,观察治疗前后免疫功能指标及癌抗原19-9(CA19-9)、癌胚抗原(CEA)水平的变化,以及毒副反应发生情况。结果:联合组总有效率为57.14%,显著高于对照组的34.15%(P<0.05);联合组治疗后CD3+、CD4+、CD4+/CD8+显著高于对照组,CD8+显著低于对照组(P<0.01)。治疗后,两组CA19-9、CEA水平均较治疗前明显降低(P<0.01),且联合组降低程度显著大于对照组(P<0.01)。联合组白细胞减少、血小板减少、周围神经毒性、血红蛋白减少、恶心呕吐等毒副反应发生率均显著低于对照组,差异有统计学意义(P<0.05)。结论:复方苦参注射液辅助治疗晚期胃癌疗效显著,毒副反应轻微,不仅可增强免疫功能,还可降低血清CA19-9、CEA水平。

Objective ：To explore the effect of Compound Kushen Injection in adjuvant chemotherapy for patients with advanced gastric cancer. Methods ：A total of 83 patients with advanced gastric cancer were randomly divided into combination group （ n = 42 ） and control group （ n = 41 ） by random number table. Both groups received modified chemotherapeutic protocol using FOLFOX6, on which basis combination group was added with Compound Kushen Injection. Clinical efficacy was compared,and the changes of levels of immunological function indexes, carbohydrate antigen 19-9 （CA19-9） and carcinoembryonic antigen （CEA） before and after treatment as well as the occurrence rate of toxic and adverse reactions were observed in both groups. Results ：The total response rate （RR） was 57. 14% in combination group, evidently higher than the 34.15 % in control group （P 〈 0.05 ）. Combination group was markedly higher in CD3 ＋ , CD4 ＋ and CD4 ＋/CD8＋ but evidently lower in CD8 ＋than control group after treatment （P 〈 0.01 ）. After treatment, levels of CA19-9 and CEA decreased notably in both groups than treatment before （P 〈 0.01 ）,which decreased more significantly in combi- nation group than that in control group （P 〈 O. 01 ）. Meanwhile, combination group was significantly lower than control group in the oc- currence rates of toxic and adverse reactions like leucopenia, thrombocytopenia,peripheral nerve toxicity,decreased hemoglobin as well as nausea and vomiting （ P 〈 0.05 ）. Conclusion ： Application of Compound Kushen Injection in adjuvant chemotherapy has excellent efficacy in the treatment of advanced gastric cancer with mild toxic and adverse reactions, and it cannot only enhance immunological function,but also reduce the levels of serum CA19-9 and CEA.
