武陵山区绝经综合征患者戊酸雌二醇片补充治疗依从性的影响因素

Influencing factors of treatment compliance of estradiol valerate pills among postmenopausal women in Wuling mountain area

目的探讨研究武陵山区绝经综合征患者戊酸雌二醇片补充治疗依从性影响因素。方法对2013年3月-2015年3月潍坊市妇幼保健院收治的110例绝经综合征患者的病历资料进行回顾性调查分析,所有患者均行戊酸雌二醇片补充治疗,对影响治疗依从性的因素进行回归分析。结果单因素分析发现,影响程度由高到低依次是:经济收入、医源性因素、对戊酸雌二醇片的认知、担心发生不良反应、家庭支持、文化程度、取药条件、心理状态;对以上单因素进行多因素Logistic回归分析,发现家庭支持(OR=3.248)是保护因素,而经济收入低(OR=4.703)、医源性差(OR=4.265)、对戊酸雌二醇片的认知偏差(OR=4.061)、担心发生不良反应(OR=3.896)是独立危险因素。结论对武陵山区绝经综合征采用戊酸雌二醇片补充治疗患者需要改善医源性条件,加强对该药物的认知宣教,鼓励家庭成员对患者进行支持,以提高其依从性。

Objective To explore the influencing factors of treatment compliance of estradiol valerate pills among postmenopausal women in Wuling mountain area. Methods The data of 110 patients with menopausal syndrome treated in Weifang Municipal Maternal and Child Health Care Hospital from March 2013 to March 2015 was surveyed and analyzed retrospectively. All the patients were treated by estradiol valerate pills. The influencing factors of treatment compliance were analyzed by regression analysis. Results Univariate analysis showed that the influencing factors of treatment compliance from high to low included economic income,iatrogenic factor,the cognition of estradiol valerate pills, worrying about adverse reactions, family support, culture degree, drug delivery conditions, and psychological status. Multivariate logistic regression analysis showed that family support（ OR = 3. 248） was a protective factor,while low economic income（ OR= 4. 703）,bad iatrogenic factor（ OR= 4. 265）,cognitive bias of estradiol valerate pills（ OR = 4. 061）,worrying about adverse reactions（ OR = 3. 896） were independent risk factors. Conclusion To improve the treatment compliance of estradiol valerate pills among postmenopausal women in Wuling mountain area,the medical conditions should be improved,cognition,propaganda and education of estradiol valerate pills should be enhanced,family support of family members should be encouraged.
