无症状高尿酸血症患者的肝、肾功和血脂水平分析 被引量:9

摘要 目的分析高尿酸血症(HUA)患者血浆肝功、肾功和血脂等生化指标,并探讨其可能的临床意义。方法选取2008年1月至2016年6月南充市中心医院体检中心38 064例成年体检者中,筛选出无症状HUA患者1 969例,同时选取年龄、性别匹配的血尿酸(UA)水平正常的健康体检者350例,测定患者体质量指数(BMI)、血压、肝功、肾功、血脂、血糖等生化指标,并分析各指标间的相关性。结果无症状HUA患者的BMI、舒张压(DBP)、UA、γ-谷氨酰转移酶(GGT)、天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、碱性磷酸酶(ALP)、血浆总蛋白(TP)、肌酐(CREA)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、血清同型半胱氨酸(HCY)和血浆葡萄糖(GLU)水平明显高于对照组,而血浆高密度脂蛋白胆固醇(HDL-C)水平明显低于对照组,差异有统计学意义(P<0.05)。男性HUA患者的UA、GGT、ALT、总胆红素(TBIL)、CREA、TG和HCY水平明显高于女性HUA患者,差异有统计学意义(P<0.05)。HUA患者UA水平与血浆GGT、AST、ALT、CREA、TG、HCY浓度呈明显正相关(P<0.05),而与血浆HDL-C浓度呈明显负相关(P<0.05)。结论无症状HUA患者的肝功、肾功、血糖和血脂的代谢状态与UA正常者明显不同,而血浆GGT活性可能是反应其体内氧化应激水平的指标。 Objective To investigate the biochemical parameters including the levels of liver function, kidney function, blood fat in hyperuricemia patients and analyze their clinical significances. Methods 1969 patients with hyperuricemia were selected as research objects from the 38,064 adults who had physical checkup in Medical Examination Centre of Nanchong Centre Hospital from January of 2008 to June of 2016, and the other 350 healthy examinees with the corresponding age were selected and divided into the control group. The body mass index (BMI) and the biochemical parameters including the levels of blood pressure, liver function, kidney function, blood fat and blood glucose were detected, and the correlation was explored between blood uric acid and the biochemical markers. Results Compared the healthy examinees in the control group, The hyperuricemia patients had higher levels in BMI, diastolic blood pressure (DBP), gamma-glutamyl transferase (GGT), aspartate aminotransferase (AST), alanine transminase (ALT), alkaline phosphatase (ALP), total plasma protein, creatinine (CREA), triglyceride (TG), low density lipoprotein cholestrol (LDL-C), homocysteine (HCY) and plasma glucose and low levels in high density lipoprotein cholestrol (HDL-C), and the differences were statistically significant (P〈0.05). The male hyperuricemia patients had higher levels in uric acid, GGT, ALT, total bilirubin (TBIL), CREA, TG, and HCY than the female ones, and the differences were statistically significant (P〈0.05). The level of blood uric acid was positively correlated with the concentration of GGT, AST, ALT, CREA, TG, and negatively correlated with the that of HDL-C. Conclusion The levels of liver function, kidney function, blood glucose, and blood fat in the asymptomatic hyperuricemia patients are different from those in the healthy persons, and the plasma GGT could be a marker of oxidative stress in hyperuricemia patients.
